经脐单切口腹腔镜阑尾切除术治疗急性阑尾炎的效果评价

Evaluation of Transumbilical Single Incision Laparoscopic Appendectomy in the Treatment of Acute Appendicitis

目的:评价经脐单切口腹腔镜阑尾切除术治疗急性阑尾炎患者的效果。方法:选取本院2018年7月-2019年12月收治的急性阑尾炎患者104例行回顾性研究,其中2018年7月-2019年3月行传统三孔腹腔镜阑尾切除术的51例患者作为传统组,2019年4-12月行经脐单切口腹腔镜阑尾切除术的53例患者作为单切口组,对比两组手术时长、术中失血量、住院时长、中转开腹率、排气时间、疼痛度(VAS评分)、美观度、并发症发生情况。结果:单切口组手术时长长于传统组,术中失血量低于传统组(P<0.05);单切口组住院时长、排气时间短于传统组,VAS评分低于传统组,美观度高于传统组(P<0.05);单切口组并发症发生率为4.00%与传统组的12.00%对比,差异无统计学意义(P>0.05)。结论:与传统三孔腹腔镜阑尾切除术治疗相比,经脐单切口腹腔镜阑尾切除术治疗急性阑尾炎,可改善手术效果以及康复效果,且并发症少。

Objective:To evaluate the effect of transumbilical single incision laparoscopic appendectomy in the treatment of acute appendicitis.Method:A total of 104 patients with acute appendicitis treated in our hospital from July 2018 to December 2019 were selected as the objects of this retrospective study.From July 2018 to March 2019,51 cases treated with traditional three-hole laparoscopic appendectomy were the traditional group.From April to December 2019,53 cases treated with transumbilical single incision laparoscopic appendectomy were the single incision group.The operation time,intraoperative blood loss,hospitalization time,conversion rate to laparotomy,exhaust time,pain(VAS score),beautiful degree and complications rate of the two groups were compared.Result:The operation duration of the single incision group was longer than that of the traditional group,and the intraoperative blood loss was lower than that of the traditional group(P<0.05).The time of hospitalization and exhaust in the single-incision group were shorter than that of the traditional group,the VAS score was lower than that of the traditional group,and the beautiful degree was higher than that of the traditional group(P<0.05).The complication rate of the single incision group were 4.00%compared with 12.00%of the conventional group,and there was no significant difference(P>0.05).Conclusion:Compared with the traditional three-hole laparoscopic appendectomy for acute appendicitis,the transumbilical single incision laparoscopic appendectomy can improve the surgical effect and recovery effect with fewer complications.
