鹰嘴为中心肘周皮下注射治疗血液透析贫血患者的效果

Effect of Subcutaneous Injection into the Elbow around the Olecranon on Hemodialysis Patients with Anemia

目的:探讨鹰嘴为中心肘周皮下注射促红细胞生成素治疗血液透析贫血患者的效果。方法:将2017年3月-2019年3月在笔者所在医院治疗的96例血液透析贫血患者作为研究对象,采用随机数字表法分为两组,对照组采用三角肌皮下注射,研究组采用鹰嘴为中心肘周皮下注射。比较两组疼痛情况、舒适情况、配合情况、注射不良反应及不同时间血色素达标情况。结果:研究组注射疼痛程度轻于对照组,舒适程度及配合程度均高于对照组,差异均有统计学意义(P<0.05)。研究组注射部位感染、硬结、出血、皮肤瘀斑发生率均低于对照组,差异均有统计学意义(P<0.05)。两组干预3、6、12个月时血色素达标率比较差异无统计学意义(P>0.05)。结论:鹰嘴为中心肘周皮下注射促红细胞生成素治疗血液透析贫血患者相较于三角肌皮下注射而言患者疼痛程度更轻,局部副作用少,是一种高效、舒适、安全的注射方法。

Objective:To investigate the effect of subcutaneous injection of Erythropoietin(EPO)into the elbow around the olecranon on hemodialysis patients with anemia.Method:A total of 96 hemodialysis anemia patients treated in the author’s hospital from March 2017 to March 2019 were studied,they were divided into two groups using the random number table method,the control group was subcutaneously injected into deltoid muscle,and the study group was subcutaneously injected into the elbow around the olecranon.The pain,comfort,coordination,adverse reactions of injection and hemoglobin standard at different times were compared between the two groups.Result:The injection pain degree in the study group was less than that in the control group,and the comfort degree and coordination degree were higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of injection site infection,induration,bleeding and skin ecchymosis in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference between the two groups in the hemoglobin attainment rate at 3,6 and 12 months of intervention(P>0.05).Conclusion:Subcutaneous injection of Erythropoietin into the elbow around the olecranon is an efficient,comfortable and safe injection method for hemodialysis patients with less pain and less local side effects than subcutaneous injection of deltoid muscle.
