益气养阴方辅助铂类为主联合化疗方案治疗气阴两虚型晚期非小细胞肺癌临床研究

Clinical Study on Yiqi Yangyin Prescription Combined with Platinum-Based Chemotherapy Regimen for Advanced Non-Small Cell Lung Cancer of Qi-Yin Deficiency Type

目的:观察益气养阴方辅助铂类为主联合化疗方案治疗气阴两虚型晚期非小细胞肺癌(NSCLC)的临床疗效。方法:选取178例气阴两虚型晚期NSCLC患者,根据不同的治疗方法分为对照组和试验组各89例。2组均给予铂类为主联合化疗方案治疗,试验组在此基础上加用益气养阴方治疗,以21 d为1个周期,2组均治疗3个周期。比较2组中医证候积分、T淋巴细胞亚群指标、卡氏评分(KPS)、实体瘤疗效及毒副反应发生情况。结果:治疗后,试验组咳嗽、咯痰、胸痛、气短、神疲乏力、口干、便秘积分均低于对照组(P<0.05),KPS高于对照组(P<0.05)。治疗后,试验组KPS升高率高于对照组(P<0.05),KPS下降率低于对照组(P<0.05)。治疗后,试验组CD3^+、CD4^+水平均高于对照组(P<0.05),CD4^+/CD8^+值大于对照组(P<0.05)。试验组有效率为52.81%,高于对照组的31.46%(P<0.05)。试验组白细胞减少、血红蛋白下降、血小板减少、胃肠道反应及肝功能损伤发生率均低于对照组(P<0.05)。结论:益气养阴方辅助铂类为主联合化疗方案治疗气阴两虚型晚期NSCLC,可有效缓解患者的临床症状、缩小瘤灶,提高其免疫功能及生活质量,减轻化疗毒副反应。

Objective:To observe the clinical effect of Yiqi Yangyin prescription combined with platinum-based chemotherapy regimen for advanced non-small cell lung cancer(NSCLC)of qi-yin deficiency type.Methods:A total of 178 cases of advanced NSCLC patients of qi-yin deficiency type were selected and divided into the control group and the experiment group according to different treatments,89 cases in each group.Both groups were given platinum-based chemotherapy regimen,and the experiment group was additionally given Yiqi Yangyin prescription,21 days being a cycle,three cycles in total.The Chinese medicine symptom scores,the indexes of T lymphocyte subsets,the scores of Karnofsky performance status(KPS),the curative effect of solid tumors and the incidence of toxic side effects were compared between the two groups.Results:After treatment,the scores of cough,expectoration,chest pain,shortness of breath,lassitude of spirit and lack of strength,dry mouth and constipation in the experiment group were lower than those in the control group(P<0.05),and the KPS was higher than that in the control group(P<0.05).After treatment,the rising rate of the KPS in the experiment group was higher than that in the control group(P<0.05),and the decline rate of the score was lower(P<0.05).After treatment,the levels of CD3^+、CD4^+ and the CD4^+/CD8^+ in the experiment group were higher than those in the control group(P<0.05).The effective rate was 52.81% in the experiment group,higher than that of 31.46% in the control group(P<0.05).The incidence of the decrease of leukocytes,hemoglobin,thrombocytopenia,gastrointestinal reaction and the liver function injury in the experiment group were lower than those in the control group(P<0.05).Conclusion:The therapy of Yiqi Yangyin prescription combined with platinum-based chemotherapy regimen in treating advanced NSCLC of qi-yin deficiency type,can effectively relieve patients’clinical symptoms,reduce tumor focus,increase immune function and quality of life of patients,and alleviate toxic side effects of chemotherapy.
