摘要
目的探讨炎琥宁治疗挤压综合征的效果及潜在作用机制。方法将48只SD大鼠随机分为对照组、挤压组、补液组、炎琥宁组,每组12只。除对照组外,其余组均应用挤压平台进行造模,挤压质量设为5 kg,挤压时间设定为16 h。解压后即刻,炎琥宁组通过尾静脉注射0.2%炎琥宁注射液5 mL/kg,补液组注射等体积生理盐水。解压12 h后取血检测血清尿素氮(BUN)、肌酐(Cr)、钾离子(K+)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、核因子-κB(NF-κB)水平,取肾组织采用实时荧光定量PCR法检测NF-κB mRNA表达水平。结果挤压组大鼠血清BUN、Cr、K+和IL-1β、IL-6、NF-κB水平均明显高于对照组(P均<0.05);补液组及炎琥宁组血清BUN、Cr、IL-1β、IL-6水平和炎琥宁组NF-κB、K+水平均明显低于挤压组(P均<0.05),且炎琥宁组血清BUN、Cr、IL-1β、IL-6、NF-κB水平均明显低于补液组(P均<0.05)。挤压组大鼠肾组织中NF-κB mRNA相对表达水平明显高于对照组(P<0.05),补液组及炎琥宁组明显低于挤压组(P<0.05),且炎琥宁组明显低于补液组(P<0.05)。结论炎琥宁可改善挤压综合征大鼠肾功能,抑制炎症因子表达,其作用机制可能与NF-κB通路相关。
Objective It is to explore the effect of Yanhuning in the treatment of crush syndrome and its potential mechanism.Methods Forty-eight SD rats were randomly divided into control group,extrusion group,liquid infusion group and Yanhuning group,12 rats in each group.Except for the control group,the other groups were all molded with an extrusion platform,the extrusion mass was set to 5 kg,and the extrusion time was set to 16 h.Immediately after decompression,the Yanhuning group was injected with 0.2%Yanhuning injection 5 mL/kg through the tail vein,and the liquid infusion group was injected with an equal volume of normal saline.12 hours after decompression,the blood of the rats was collected to detect the levels of serum urea nitrogen(BUN),creatinine(Cr),potassium ion(K+),interleukin-1β(IL-1β),interleukin-6(IL-6)and nuclear factor-κB(NF-κB),kidney tissues were taken to detect NF-κB mRNA expression level by real-time fluorescent quantitative PCR method.Results The levels of serum BUN,Cr,K+,IL-1β,IL-6,and NF-κB in the extrusion group were significantly higher than those in the control group(all P<0.05);the levels of serum BUN,Cr,IL-1β,IL-6 levels in the fluid supplement group and Yanhuning group and the levels of NF-κB and K+in the Yanhuning group were significantly lower than those of the extrusion group(all P<0.05),and the levels of serum BUN,Cr,IL-1β,IL-6,NF-κB were significantly lower than those in the fluid supplement group(all P<0.05).The relative expression level of NF-κB mRNA in the kidney tissue of the extrusion group was significantly higher than that of the control group(P<0.05),and the fluid supplement group and Yanhuning group were significantly lower than the extrusion group(P<0.05),and the Yanhuning group was lower than the fluid supplement group(P<0.05).Conclusion Yanhuning can improve the renal function of rats with crush syndrome and inhibit the expression of inflammatory factors.Its mechanism of action may be related to the NF-κB pathway.
