摘要
目的:研究乳腺钼靶检查的临床应用价值及应用限度。方法:回顾性分析2015年1月-2019年11月笔者所在医院进行乳腺钼靶检查且具有手术病理结果的318例患者临床资料。比较不同分型乳腺的BI-RADS分级与术后病理结果的诊断准确率。结果:318例乳腺钼靶检查患者中,79例为致密型乳腺,173例为中间混合型乳腺,66例为脂肪型乳腺。根据美国放射学分会推荐的BI-RADS标准进行分级,其中有215例患者为Ⅱ~Ⅲ级,有54例患者为Ⅳ级,有49例患者为Ⅴ级。318例患者手术病理结果显示,良性214例,恶性104例;致密型乳腺BI-RADS分级、中间混合型乳腺BI-RADS分级、脂肪型乳腺B-IRADS分级、有钙化灶的乳腺B-IRADS分级同术后病理检查诊断准确率分别为81.01%、91.91%、93.94%、95.52%,同术后病理检查一致性较好,Kappa值分别为0.702、0.855、0.874、0.886。结论:乳腺钼靶检查诊断乳腺疾病的诊断结果与术后病理结果相比一致性高,值得临床推广应用。
Objective:To study the clinical application value and application limit of breast molybdenum target examination.Method:The clinical data of 318 patients with breast molybdenum target examination and surgical pathological results in our hospital from January 2015 to November 2019 were analyzed retrospectively.The diagnostic accuracy rate between the BI-RADS grading of different breast types and postoperative pathological results were compared.Result:Among the 318 breast molybdenum target examination patients,79 were dense type,173 were intermediate mixed breast,and 66 were fat breast.According to the American college of radiology branch recommend BI-RADS grading standards,there were 215 patients of gradeⅡtoⅢ,54 patients of gradeⅣ,49 patients of gradeⅤ.The surgical pathological results of 318 patients showed that 214 cases were benign and 104 cases were malignant.Compared with postoperative pathological examinations,diagnostic accuracy rates of BI-RADS grading of dense breast,BI-RADS grading of intermediate mixed breast,BI-RADS grading of fat breast and BI-RADS grading of calcified breast were 81.01%,91.91%,93.94%and 95.52%,respectively,which were consistent with postoperative pathological examination.The Kappa values were 0.702,0.855,0.874,and 0.886,respectively.Conclusion:The diagnostic results of the breast molybdenum target examination are consistent with the postoperative pathological results,which is worthy of widespread clinical application.
