高血压患者血浆同型半胱氨酸对颈动脉内膜中层厚度的影响

摘要 目的:研究高血压患者血浆同型半胱氨酸(Hcy)对颈动脉内膜中层厚度(IMT)的影响。方法:选取2018年1月-2019年1月笔者所在医院接收的100例高血压患者作为研究对象,以患者Hcy水平高低分为对照组(n=50,Hcy正常)及试验组(n=50,Hcy增高),采集100例患者临床基础资料、Hcy水平及IMT,分析高血压患者Hcy对IMT的影响。结果:两组年龄、吸烟情况、糖尿病、LDL-C、收缩压、舒张压及IMT对比,差异均有统计学意义(P<0.05);多因素分析得出,影响Hcy水平升高的危险因素有年龄高、吸烟、合并糖尿病、LDL-C升高、血压升高、IMT增厚,差异均有统计学意义(P<0.05);Spearman程序相关性分析得出,IMT与Hcy呈正相关(r=0.581,P<0.05)。结论:颈动脉内膜中层厚度随着Hcy水平的升高逐渐增厚,提高动脉硬化发生率,可将Hcy水平作为预测IMT的指标。 Objective:To study the effect of plasma homocysteine(Hcy)on carotid intima-media thickness(IMT)in patients with hypertension.Method:A total of 100 patients with hypertension from January 2018 to January 2019 were selected as the research objects.Patients were divided into control group(n=50,normal Hcy)and experimental group(n=50,increased Hcy)according to their Hcy level.Clinical basic data,Hcy level and IMT of 100 patients were collected to analyze the influence of Hcy on IMT in hypertensive patients.Result:There were statistically significant differences in age,smoking status,diabetes mellitus,LDL-C,systolic blood pressure,diastolic blood pressure and IMT between the two groups(P<0.05).Multi-factor analysis showed that the risk factors affecting the increase of Hcy level were high age,smoking,diabetes mellitus,elevated LDL-C,elevated blood pressure,and IMT thickening,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that IMT was positively correlated with Hcy(r=0.581,P<0.05).Conclusion:The intima-media thickness of carotid artery gradually increases with the increase of Hcy level,which can be used as an index to predict IMT.
