妊娠中期行腹腔镜手术治疗妊娠合并卵巢肿瘤的可行性及效果分析

The Feasibility and Effect of Laparoscopic Surgery in the Second Trimester of Pregnancy Complicated with Ovarian Tumor

目的:研究妊娠中期行腹腔镜手术治疗妊娠合并卵巢肿瘤的可行性及效果。方法:纳入2016年4月-2019年3月笔者所在医院收治的76例妊娠合并卵巢肿瘤患者展开研究,按照随机数字表法分为两组,对照组38例实施开腹手术治疗,观察组38例采用腹腔镜手术治疗。对比两组围术期相关指标、手术成功率、术后并发症发生率、妊娠结局。结果:观察组术后出血量少于对照组,手术时间、术后排气时间及住院时间短于对照组,疼痛评分低于对照组,差异均有统计学意义(P<0.05);观察组手术成功率高于对照组,术后并发症发生率、剖宫产率、早产率、新生儿窒息发生率均低于对照组,差异均有统计学意义(P<0.05)。结论:在妊娠中期对妊娠合并卵巢肿瘤患者实施腹腔镜手术治疗具有较高的手术成功率及安全性,可获得良好的妊娠结局,临床可行性较高。

Objective:To study the feasibility and effect of laparoscopic surgery in the second trimester of pregnancy complicated with ovarian tumor.Method:A total of 76 patients with pregnancy complicated with ovarian tumor admitted to our hospital from April 2016 to March 2019 were included in the study.They were divided into two groups according to the random number table,38 cases in the control group were treated with open surgery,38 cases in the observation group were treated with laparoscopic surgery.The indexes of perioperative period,the success rate of operation,the incidence of postoperative complications and the outcome of pregnancy were compared between the two groups.Result:The amount of postoperative bleeding in the observation group was less than that in the control group,the operation time,postoperative exhaust time and hospital stay in the observation group were shorter than those in the control group,and the pain score in the observation group was lower than that in the control group,the differences were statistically significant (P<0.05).The success rate of operation in the observation group was higher than that in the control group,and the incidence of postoperative complications,cesarean section,premature delivery and neonatal asphyxia in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Laparoscopic surgery in the second trimester of pregnancy for patients with ovarian tumor has a high success rate and safety,can achieve a good pregnancy outcome,clinical feasibility is high.
