胫骨横向骨搬移治疗糖尿病足的讨论

Discussion on the Treatment of Diabetic Foot with Lateral Tibial Bone Transfer

目的探析胫骨横向骨搬移治疗糖尿病足的效果。方法收集该院2016年9月—2019年6月时段收治糖尿病足患者20例,随机分为普通换药组(5例,给予普通换药治疗)、胫骨横向骨搬移治疗组(8例,给予胫骨横向骨搬移治疗)、结合中药治疗组(7例,胫骨横向骨搬移结合中医药治疗),记录3组足趾坏死发生情况,对比3组疼痛情况、创面愈合时间、皮温。结果结合中药治疗组VAS评分较普通换药组更低(P<0.05),与胫骨横向骨搬移治疗组对比差异无统计学意义(P>0.05),皮温为结合中医治疗组最高,明显高于普通换药组(P<0.05),但与胫骨横向骨搬移治疗组差异无统计学意义(P>0.05),创面愈合时间为结合中药治疗组明显短于胫骨横向骨搬移治疗组、普通换药组(P<0.05)。普通换药组足趾坏死发生率明显高于结合中药治疗组及胫骨横向骨搬移治疗组(P<0.05)。结论糖尿病足治疗中落实胫骨横向骨搬移治疗效果显著,结合中医药治疗可进一步巩固疗效,可积极推广。

Objective To explore the effect of lateral tibial bone transfer on diabetic foot.Methods Collect 20 cases of diabetic foot patients in our hospital from September 2016 to June 2019,and randomly divide them into a general dressing group(5 cases,given general dressing treatment)and a tibial lateral bone transfer treatment group(8 cases,given tibial lateral bone transfer treatment).The traditional Chinese medicine treatment combination group(7 cases,lateral tibial bone movement combined with traditional Chinese medicine treatment),record the occurrence of toe necrosis in the three groups,and compare the pain situation,wound healing time,and skin temperature in the three groups.Results The VAS score of the combined Chinese medicine treatment group was lower than that of the common dressing group(P<0.05),and there was no statistically significant difference compared with the tibial lateral bone migration treatment group(P>0.05).The skin temperature was the highest in the combined Chinese medicine treatment group,which was significantly higher than that in the common dressing group(P<0.05),but there was no statistically significant difference from the tibial lateral bone transfer treatment group(P>0.05).The wound healing time was significantly shorter in the combined traditional Chinese medicine treatment group than the tibial lateral bone transfer treatment group and the common dressing group(P<0.05).The incidence of toe necrosis in the common dressing group was significantly higher than that in the combined Chinese medicine treatment group and the tibial lateral bone migration treatment group(P<0.05).Conclusion In the treatment of diabetic foot,the effect of lateral tibial bone migration is significant.Combined with traditional Chinese medicine treatment,it can further consolidate the effect and can be actively promoted.
