雷贝拉唑四联疗法治疗幽门螺杆菌阳性消化性溃疡的临床刍议

Rhebeprazole Quadruple Therapy for the Treatment of Helicobacter Pylori Positive Peptic Ulcer

目的阐述幽门螺杆菌呈阳性的消化性溃疡患者采用雷贝拉唑四联法治疗的临床应用效果。方法该次研究时间为2018年6月—2019年6月,研究对象选定黔东南州人民医院收治的消化性溃疡患者90例,经检验幽门螺杆菌呈阳性,随机分为两组,对照组采用常规枸橼酸铋钾颗粒、阿莫西林、呋喃唑酮三联法治疗,观察组在其基础上加用雷贝拉唑行四联法治疗,对比两组临床总疗效及HP转阴率,记录分析各组患者临床相关指标差异,计算各组患者用药后发生不良反应的概率。结果观察组消化溃疡患者治疗总有效率、HP转阴率均明显高于对照组(χ~2=6.050、6.150,P<0.05)。同时,观察组患者溃疡面愈合时间(10.13±1.22)d相较对照组更短,腹痛症状评分(2.01±0.36)分、反酸症状评分(2.05±0.21)分、嗳气症状评分(1.49±0.17)分相较对照组更低(t=8.460、8.940、9.010、8.830,P<0.05)。两组患者治疗后均未出现严重不良反应,总发生率相近(χ~2=0.000,P>0.05)。结论幽门螺杆菌呈阳性的消化性溃疡患者采用雷贝拉唑四联法治疗效果确切,可有效改善患者症状表现,促进溃疡面愈合,且安全性好。

Objective To elaborate the clinical application effect of rapeprazole quadruple therapy in patients with peptic ulcer positive for H.pylori.Methods The time of this study was from June 2018 to June 2019.The study subjects selected 90 patients with peptic ulcer admitted to the People’s Hospital of Southeast Guizhou Prefecture,and tested positive for Helicobacter pylori,randomly divided into two groups:the control group was treated with the conventional triple therapy of bismuth potassium citrate granules,amoxicillin and furazolidone,and the observation group was treated with a combination of rabeprazole and quadruple therapy on the basis of it,to record and analyze the differences of clinical related indexes of patients in each group,and calculate the probability of adverse reactions after medication in each group.Results The total effective rate of treatment and the rate of HP to negative conversion in patients with peptic ulcer in observation group were significantly higher than those in control group(χ~2=6.050,6.150,P<0.05).At the same time,the healing time of the ulcer surface in the observation group(10.13±1.22)d was shorter than that in the control group,abdominal pain symptom score(2.01±0.36)points,acid reflux symptom score(2.05±0.21)points,belching symptom score(1.49±0.17)points,lower than the control group(t=8.460,8.940,9.010,8.830,P<0.05).After treatment in both groups,serious adverse reactions occurred and the total incidence was similar(χ~2=0.285,P>0.05).Conclusion The treatment of patients with peptic ulcer with positive Helicobacter pylori using rabeprazole quadruple method has a definite effect.It can effectively improve the patient’s symptom performance,promote healing of ulcer surface,and has good safety.
