下腹部腹膜皱襞在儿童腹股沟疝手术治疗中的应用现状及技术要点

Application Status and Technical Points of Hypogastric Navel Crenulation in Children with Inguinal Hernia

下腹部腹膜皱襞共5条,其中脐正中襞1条,脐内侧襞和脐外侧襞各2条,均可作为独立解剖结构应用于儿童腹股沟疝的腹腔镜治疗。儿童腹股沟疝手术治疗方法主要有3种①将腹膜皱襞直接缝合于前外侧腹壁覆盖加强疝环口;②制作带蒂腹膜瓣片覆盖加强疝环口;③制作带蒂腹膜瓣片填塞疝环口。根据具体情况妥善应用上述方法可有效预防儿童腹股沟疝术后复发。综述了下腹部腹膜皱襞的解剖学基础及其在儿童腹股沟疝手术治疗中的应用现状和技术要点。

Hypogastric navel folds,including one median umbilical fold,two medial umbilical folds,and two lateral umbilical folds,can be used as independent anatomical structures in the laparoscopic treatment of pediatric inguinal hernia.There are three kinds of surgical treatment for inguinal hernia in children:①the navel crenulation was sutured directly to the lateral abdominal wall to reinforce hernia orifice;②a pedicled peritoneal flap was prepared to reinforce hernia orifice;③a pedicled peritoneal flap was made to plug the hernia orifice.Appropriate application of the above methods can effectively prevent children from the recurrence of inguinal hernia.Anatomical basis of hypogastric navel crenulation and its application status and technical points in the surgical treatment of inguinal hernia in children are reviewed.
