无创超声心排血量监测结果与ICU老年心力衰竭患者心功能分级的相关性研究

Correlation between non-invasive echocardiographic output monitoring results and cardiac function classification of ICU heart failure in elderly patients

目的:研究无创超声心排血量监测结果与ICU老年心力衰竭患者心功能分级的相关性。方法:筛选2016年1月至2019年6月某院ICU收治的90例老年心力衰竭患者[美国纽约心脏病协会(New York Heart Association,NYHA)心功能分级Ⅱ级11例、Ⅲ级64例、Ⅳ级15例]为心力衰竭组、90例老年非心脏病患者(NYHA心功能分级均为Ⅰ级)为对照组。所有患者均接受无创超声心排血量监测,比较心力衰竭组与对照组患者以及心力衰竭组内不同NYHA心功能分级患者的心输出量、心脏指数和每搏输出量。采用SPSS 22.0统计软件进行数据处理,采用Spearman相关性分析法对无创超声心排血量监测结果与ICU老年心力衰竭患者心功能分级的相关性进行分析。结果:心力衰竭组心输出量、心脏指数、每搏输出量均低于对照组(P<0.05)。心力衰竭组中不同NYHA心功能分级患者的心输出量、心脏指数、每搏输出量比较差异有统计学意义(P<0.05),Ⅱ级患者的心输出量、心脏指数、每搏输出量均高于Ⅲ、Ⅳ级(P<0.05),Ⅲ级患者的心输出量、心脏指数、每搏输出量均高于Ⅳ级(P<0.05)。心输出量、心脏指数、每搏输出量与ICU老年心力衰竭患者心功能分级均呈负相关(r=-0.74、-0.75、-0.72,P均<0.05)。结论:无创超声心排血量监测结果与ICU老年心力衰竭患者心功能分级呈负相关,通过观察其数值变化有助于评估心功能分级。

Objective To study the correlation between the results of non-invasive echocardiographic output monitoring and cardiac function classification of ICU heart failure elderly patients.Methods From January 2016 to June 2019,90 elderly patients with heart failure admitted to the ICU of some hospital were enrolled into a heart failure group including 11 ones with classⅡcardiac function classified according to New York Heart Association(NYHA),64 ones with classⅢcardiac function and 15 ones with classⅣcardiac function,and another 90 with no heart diseases were selected into a control group who had classⅠcardiac function according to NYHA.All the patients underwent non-invasive echocardiographic blood output monitoring,and the cardiac output,cardiac index and output per stroke were compared between the patients in the heart failure group and the control group,as well as between the patients with different classes of NYHA cardiac functions in the heart failure group.SPSS 22.0 statistical software was used for data processing,and Spearman's correlation analysis was applied to analyzing the correlation between the results of non-invasive echocardiographic blood output monitoring and cardiac function classification of the ICU heart failure elderly patients.Results The cardiac output,cardiac index,and stroke volume of the heart failure group were lower than those of the control group,the difference was statistically significant(P<0.05).The differences in the indexes above in the heart failure group were statistically significant(P<0.05),and the indexes in the patients with NYHA classⅡcardiac function were higher than those in the patients with classⅢandⅣ,and those with classⅢwere higher than those with classⅣfunction(P<0.05).The indexes were all negatively correlated with cardiac function classification of the ICU heart failure elderly patients(r=-0.74,-0.75 and-0.72,all P<0.05).Conclusion The results of noninvasive echocardiography are negatively correlated with the occurrence of heart failure in ICU,and it is helpful to classify cardiac functions by observing the changes of their values.
