摘要
目的探讨环状RNA hsacirc0002715在系统性红斑狼疮(SLE)患者诊断及鉴别诊断中的应用价值。方法应用qRT-PCR检测76例SLE患者、33例健康对照者和88例非SLE患者(56例类风湿性关节炎患者和32例强直性脊柱炎患者)全血环状RNA hsacirc0002715表达水平,分析SLE患者全血环状RNA hsacirc0002715表达水平与临床指标的相关性,并采用受试者工作曲线(ROC)评价全血环状RNA hsacirc0002715诊断价值。结果①SLE患者全血环状RNA hsacirc0002715表达水平均高于健康对照者(P<0.001)和非SLE患者(P<0.001),差异有统计学意义。②SLE患者全血环状RNA hsacirc0002715表达水平与补体3(rs=-0.453,P<0.001),补体4(rs=-0.336,P=0.004),白细胞数量(rs=-0.486,P<0.001),血红蛋白(rs=-0.277,P<0.016),血小板数量(rs=-0.300,P=0.009)呈负相关,与抗双链DNA抗体(rs=0.253,P=0.040)呈正相关,抗核小体阳性的SLE患者其全血环状RNA hsacirc0002715表达水平高于阴性者(P<0.001)。③SLE患者和健康对照者全血环状RNA hsacirc0002715 ROC曲线下面积(AUC)为0.772,敏感性为63.16%,特异性为81.82%。SLE患者和非SLE患者全血环状RNA hsacirc0002715 AUC为0.674,敏感性为55.26%,特异性为72.73%;SLE患者和所有对照者(非SLE患者和健康对照者)全血环状RNA hsacirc0002715 AUC为0.701,敏感性为51.32%,特异性为80.17%。④环状RNA hsacirc0002715对SLE的诊断敏感性高于anti-dsDNA,且联合环状RNA hsacirc0002715-anti-dsDNA对SLE的诊断效能升高。结论SLE患者全血环状RNA hsacirc0002715的表达明显上调,且与SLE患者的疾病活动性和血液系统受累严重程度密切相关,联合hsacirc0002715-anti-dsDNA可作为SLE的潜在诊断及鉴别诊断标志物。
Objective To investigate the role of peripheral blood circular RNA hsacirc0002715 in the diagnosis and differential diagnosis of systemic lupus erythematosus(SLE)patients.Methods Peripheral blood samples were collected from 76 SLE patients,33 healthy controls(HC)and 88 non-SLE patients including 56 rheumatoid arthritis(RA)patients and 32 ankylosing spondylitis(AS)patients.Expression of peripheral blood circular RNA hsacirc0002715 was detected by qRT-PCR.Differences in peripheral blood circular RNA hsacirc0002715 among SLE,HC and non-SLE groups were comparatively analyzed.Correlations of peripheral blood circular RNA hsacirc0002715 with several clinical indexes were analyzed.Results①The expression of peripheral blood circular RNA hsacirc0002715 was significantly higher than that in HC(P<0.001)and non-SLE(P<0.001).②The expression of peripheral blood circular RNA hsacirc0002715 in SLE patients was negatively correlated with C3(rs=-0.453,P<0.001),C4(rs=-0.336,P=0.004),WBC(rs=-0.486,P<0.001),HGB(rs=-0.277,P<0.016),PLT(rs=-0.300,P=0.009).The expression of peripheral blood circular RNA hsacirc0002715 in SLE patients was positively correlated with anti-ds DNA(rs=0.253,P=0.040).The expression of peripheral blood circular RNA hsacirc0002715 in SLE patients with postive anti-nucleosome was significantly higher than that in SLE patients with negative anti-nucleosome(P<0.001).(3)The area under ROC curve(AUC),sensibility and specificity of peripheral blood circular RNA hsa circ 0002715 between SLE patients and HC was 0.772,63.16%,81.82%.The AUC,sensibility and specificity of peripheral blood circular RNA hsacirc0002715 between SLE patients and non-SLE was 0.674,55.26%,72.73%.The AUC,sensibility and specificity of peripheral blood circular RNA hsa circ 0002715 between SLE patients and all controls(non-SLE+HC)was 0.701,51.32%,80.17%.(4)The sensibility of peripheral blood circular RNA hsacirc0002715 for SLE patients was superior to anti-ds DNA,and the sensibility and sensibility of combination of hsacirc0002715-anti-ds DNA significantly increased.Conclusion The expression of peripheral blood circular RNA hsacirc0002715 significantly increases in SLE patients,and it is correlated with the activity of SLE and damage of hematological system.This study suggests that the peripheral blood circular RNA hsacirc0002715 may be a potential diagnosis and differential diagnosis biomarker for SLE.
