中医护理在危重症患者早期肠内营养中的应用 被引量:20

摘要 目的探讨中医护理在危重症患者早期肠内营养中的应用效果。方法选取2017年1月~2019年10月在广州市番禺区中医院重症监护室(ICU)接受早期肠内营养支持的88例危重症患者作为研究对象,依据随机数表法将患者分为干预组和常规组,每组44例。常规组患者采用常规护理方案,干预组患者在常规护理的基础上采用中医护理方案实施干预。经肠内营养支持12 d后,比较两组患者护理前后的营养生理指标、并发症及内环境变化情况。结果两组患者营养支持第6天的血红蛋白、血清白蛋白和血清总蛋白水平比较差异均无统计学意义(P>0.05);但干预组患者营养支持第12天的血红蛋白、血清白蛋白和血清总蛋白水平分别为(121.88±13.07)g/L、(31.37±5.06)g/L和(63.34±4.86)g/L,明显高于对照组的(110.45±12.68)g/L、(24.91±4.78)g/L和(54.91±5.42)g/L,差异均有统计学意义(P<0.05);干预组患者的总并发症发生率为11.36%,明显低于对照组的31.82%,差异有统计学意义(P<0.05);干预组患者血糖紊乱和水电解质紊乱的总发生率为6.82%,明显低于对照组的25.00%,差异有统计学意义(P<0.05)。结论中医护理在危重症患者早期肠内营养中的应用效果良好,能明显改善营养状况和内环境情况,降低胃肠道并发症的发生率。 Objective To study the effect of traditional Chinese medicine nursing on early enteral nutrition in critical patients.Methods Eighty-eight critically ill patients who received early enteral nutrition support in ICU,Guangzhou Panyu District Hospital of Traditional Chinese Medicine from January 2017 to October 2019 were selected as the research objects,which were divided into intervention group and routine group according to the random number table,with 44 patients in each group.The patients in the routine group were given routine nursing plan,while the patients in the intervention group were given traditional Chinese medicine nursing plan on the basis of routine nursing.After 12 days of enteral nutrition support,the nutritional physiological indexes,complications and internal environment of the two groups were compared before and after nursing.Results There was no significant difference in hemoglobin,serum albumin,and serum total protein levels between the two groups on the 6th day of nutritional support(P>0.05);but the hemoglobin,serum albumin,and serum total protein levels were(121.88±13.07)g/L,(31.37±5.06)mg/L,and(63.34±4.86)g/L,which were significantly better than(110.45±12.68)g/L,(24.91±4.78)mg/L,and(54.91±5.42)g/L in the control group(P<0.05);the incidence of complications in the intervention group was 11.36%,which was significantly lower than 31.82%in the control group(P<0.05);the incidence of blood glucose disorder and quality water electrolysis disorder in the intervention group was 6.82%,which was significantly lower than 25.00%in the control group(P<0.05).Conclusion The application of traditional Chinese medicine nursing in early enteral nutrition of critically ill patients is effective,which can significantly improve nutritional status and material metabolism,and reduce the incidence of gastrointestinal complications.
