临床路径护理对子宫肌瘤手术患者焦虑情绪及应激反应的影响

Effect of clinical pathway nursing on anxiety and stress response of patients undergoing hysteromyoma surgery

目的分析临床路径护理对子宫肌瘤手术患者焦虑情绪及应激反应的影响。方法选取2017年5月—2019年2月本院收治的子宫肌瘤手术患者86例,以随机数字表法分组,两组均43例。对照组提供常规护理,观察组实施临床路径护理。比较两组患者心理状态与应激反应。结果干预后观察组SAS、SDS评分[(41.24±5.24)分、(43.24±5.51)分]均比对照组[(49.72±7.33)分、(50.64±7.87)分]低,差异有统计学意义(t=6.172、5.051,P值均<0.001);术毕时观察组去甲肾上腺素、肾上腺素及皮质醇水平[(251.62±27.44)ng/L、(128.36±19.57)ng/L、(117.97±13.58)ng/L]均比对照组[(330.10±32.37)ng/L、(144.87±21.14)ng/L、(134.87±14.84)ng/L]低,差异有统计学意义(t=12.127、3.758、5.509,P值均<0.001)。结论运用临床路径护理可有效改善患者焦虑、抑郁等心理,减少应激反应。

Objective To evaluate the effect of clinical pathway nursing on anxiety and stress response of patients undergoing hysteromyoma surgery.Methods From May 2017 to February 2019,86 patients with uterine myoma were randomly divided into a control group and a trial group,with 43 patients in each group.The control group received the routine nursing,while the trial group was given the clinical pathway nursing.The psychological state and stress response of the patients in two groups were investigated and compared.Results After the intervention,the scores of SAS and SDS of the trial group were lower than those of the control group [(41.24±5.24) points and(43.24±5.51) points vs.(49.72±7.33)points and(50.64±7.87) points,t=6.172,5.051,both P=0.000].At the end of the operation,the average levels of norepinephrine,epinephrine,and cortisol of the patients in the trial group were lower than those of the patients in the control group[(251.62±27.44) ng/L,(128.36±19.57) ng/L,and(117.97±13.58) ng/L vs.(330.10±32.37) ng/L,(144.87±21.14) ng/L,and(134.87±14.84) ng/L,t=12.127,3.758,and 5.509,all P=0.000].Conclusion The clinical pathway nursing can effectively improve the anxiety and depression of patients and reduce the stress response.
