神经节苷脂钠对重症颅脑损伤病人机体应激、神经损伤程度、神经功能重建相关因子及凝血指标的影响

Effects of Ganglioside Sodium on Stress,Degree of Nerve Injury,Related Factors of Nerve Function Reconstruction and Coagulation Indexes in Patients with Severe Craniocerebral Injury

目的探讨神经节苷脂钠对重症颅脑损伤病人机体应激、神经损伤程度、神经功能重建相关因子及凝血指标的影响。方法以180例患者为对象,随机分为实验组、对照组,各90例。实验组采用神经节苷脂钠治疗,对照组采用常规法治疗。观察两组患者机体应激(即去甲肾上腺素(NE)、皮质醇(Cor)指标)、神经损伤程度(神经元特异性烯醇化酶(NSE)、星形胶质源性蛋白(S100β)、泛素羧基末端水解酶L1(UCH-L1)、胶质纤维酸性蛋白(GFAP)指标)、神经功能重建(脑源性神经营养因子(BDNF)、神经生长因子(NGF))及各项凝血功能(活化部分凝血酶原时间(aPTT)、凝血酶原时间(PT)、凝血酶时间(TT)及纤维蛋白原(Fbg)指标)。结果治疗后,实验组相比较对照组机体应激水平指标NE、Cor下降(P<0.05),神经功能损伤程度相关指标NSE、S100β、UCH-L1、GFAP水平均下降(P<0.05),神经功能重建指标BDNF、NGF水平均提升(P<0.05),两组患者治疗前各项凝血指标均呈相对高凝状态,比较无差异(P>0.05),治疗后两组患者均转向相对低凝,但实验组更明显(P<0.05)。结论神经节苷脂钠可有效改善重症颅脑损伤病人神经损伤症状,且可以缓和机体应激反应,加强神经功能重建,并可改善患者凝血指标。

Objective To investigate the effect of ganglioside sodium on stress,degree of nerve injury,related factors of nerve function reconstruction and coagulation indexes in patients with severe brain injury.Methods 180 patients were randomly divided into experimental group and control group,90 cases each.The experimental group was treated with ganglioside sodium,while the control group was treated with routine method.The stress(NE,cor),nerve injury(NSE,S100β),ubiquitin carboxy terminal hydrolase L1(UCH-L1),glial fibrillary acidic protein(GFAP),nerve function reconstruction(BDNF)were observed Nerve growth factor(NGF)and coagulation function(aPTT,PT,TT and Fbg).Results After treatment,NE and cor decreased,NSE,S100β,UCH-L1 and GFAP decreased,BDNF and NGF increased in the experimental group compared with the control group(P<0.05),The coagulation index before treatment in both groups showed a relatively high coagulation state,with no difference(P>0.05),and both groups of patients were transferred to relatively low coagulation after treatment,but the experimental group was more obvious(P<0.05)Conclusion Ganglioside sodium can effectively improve the symptoms of nerve injury in patients with severe craniocerebral injury,alleviate the stress response of the body,strengthen the reconstruction of nerve function,and improve the coagulation index of patients.
