穴位埋线防治中晚期非小细胞肺癌化疗后不良反应临床观察

Clinical Observation of Acupoint Thread-embedding in Preventing and Treating the Adverse Reactions After Chemotherapy for Advanced Non-small Cell Lung Cancer

目的观察穴位埋线防治中晚期非小细胞肺癌化疗后不良反应的临床疗效。方法 72例非小细胞肺癌中晚期患者,随机分为治疗组和对照组,每组36例。两组均采用GP化疗方案,治疗组加用穴位埋线治疗。观察两组治疗前后免疫指标变化,两组治疗后不良反应分级、卡氏评分分级,并比较两组临床疗效。结果两组治疗前免疫指标(CD3^+、CD4^+、CD8^+及CD4^+/CD8^+)比较,差异无统计学意义(P>0.05);两组治疗后CD3^+、CD4^+及CD8^+较治疗前降低(P<0.01);治疗组治疗后免疫指标优于对照组(P<0.01)。治疗后,治疗组不良反应中白细胞减少、血红蛋白减少、恶心呕吐分级与对照组比较,差异有统计学意义(P<0.01)。治疗组治疗后卡氏评分总有效率明显优于对照组(P<0.05)。治疗组疾病控制率为75.0%,明显优于对照组的50.0%(P<0.05)。结论穴位埋线对中晚期非小细胞肺癌患者化疗时出现的胃肠道不良反应有明显的预防和治疗作用,并且能够提高机体免疫功能,提高生活质量。

Objective To observe the clinical efficacy of acupoint thread-embedding in preventing and treating the adverse reactions after chemotherapy for advanced non-small cell lung cancer(NSCLC).Method Seventy-two patients with advanced NSCLC were randomized into a treatment group and a control group.The two groups both received GP chemotherapy regime.While the treatment group was given additional acupoint thread-embedding.The immune parameters before and after treatment,and the adverse reaction grading and Karnofsky Performance Score(KPS)were observed in the two groups,and the clinical efficacy was compared between the two groups.Result There were no significant differences in the immune parameters(CD3^﹢,CD4^﹢,CD8^﹢and CD4^﹢/CD8^﹢)between the two groups before treatment(P>0.05).After treatment,CD3^﹢,CD4^﹢,CD8^﹢dropped in both groups(P<0.01).The immune parameters were better in the treatment group than in the control group after intervention(P<0.01).After intervention,the treatment group had better results in the decrease of white blood cells and hemoglobin and nausea/vomiting grading compared with the control group,and the differences were statistically significant(P<0.01).The KPS-based effective rate was significantly better in the treatment group than in the control group after intervention(P<0.05).The disease control rate was 75.0%in the treatment group,significantly superior to 50.0%in the control group(P<0.05).Conclusion Acupoint thread-embedding can produce notable preventive and treatment effects on the gastrointestinal adverse reactions induced by chemotherapy in patients with advanced NSCLC,and it can enhance the immune function and improve the quality of life.
