超敏C反应蛋白、二聚体、脑利钠肽前体在房颤合并慢性心力衰竭患者的表达

Expression of hypersensitive C-reactive protein,dimer and pro-brain natriuretic peptide in patients with atrial fibrillation complicated with chronic heart failure

目的分析超敏C反应蛋白、二聚体、脑利钠肽前体在房颤合并慢性心力衰竭患者的表达。方法回顾我院2017年3月至2019年3月收治的心房颤动患者临床资料80例。按照是否合并慢性心力衰竭将其分成合并组与单纯组,例数分别为28例、52例。统计患者临床特征包括性别、年龄、冠心病、高血压、吸烟、饮酒、糖尿病及检测左心室功能(左心房内径、左室舒张末内径及左心室射血分数)患者入院后抽取肘静脉血5ml,使用全自动生化分析仪检测CRP、D-二聚体、pro-BNP水平。结果合并组患者冠心病、高血压、吸烟、饮酒、糖尿病比高于单纯组(P<0.05),两组患者性别比、年龄差异比较无统计学意义(P>0.05)。合并组患者左心房内径、左室舒张末内径大于单纯组(P<0.05),心室射血分数小于单纯组(P<0.05)。合并组患者的CRP、D-二聚体、pro-BNP水平高于单纯组(P<0.05)。左心房内径、左室舒张末内径与CRP、D-二聚体、BNP水平呈正相关。左心与室射血分数与CRP、D-二聚体、pro-BNP水平呈负相关。结论联合检测C反应蛋白、二聚体、脑利钠肽前体水平有利于预测心房颤动并心衰病情,病情越严重的患者其C反应蛋白、二聚体、脑利钠肽前体水平越高、心室功能越差,且多合并冠心病、高血压、糖尿病。

Objective To investigate the expressions of hypersensitive C-reactive protein(CRP),D-dimer and pro-brain natriuretic peptide(pro-BNP)in patients with atrial fibrillation complicated with chronic heart failure.Methods The clinical data of 80 patients with atrial fibrillation admitted to our hospital from March 2017 to March 2019 were reviewed.The patients were divided into two groups according to the presence or absence of chronic heart failure,combined group(n=28)and simple group(n=52).The clinical characteristics including gender,age,coronary heart disease,hypertension,smoking,alcohol consumption,diabetes were observed,and the left ventricular function indicators(left atrial diameter,left ventricular end diastolic diameter,and left ventricular ejection fraction)and CRP,D-dimer,pro-BNP levels in the three groups were detected.Results The proportions of coronary heart disease,hypertension,smoking,drinking and diabetes of combined group were higher than those of the single group(P<0.05),while no difference was found in gender and age(P>0.05).The left atrial diameter and left ventricular end diastolic diameter were larger in the combined group than in the simple group(P<0.05),and the ventricular ejection fraction was smaller than the simple group(P<0.05).CRP,D-dimer and pro-BNP levels were positively correlated with left atrial diameter and left ventricular end-diastolic diameter,and were negatively correlated with left ventricular ejection fraction.Conclusion Combined detection of CRP,D-dimer and pro-BNP is beneficial to the diagnosis of atrial fibrillation and heart failure,and the levels of the three are positively correlated with the disease severities.
