偏瘫患者的躯干异常姿势康复治疗对其运动及功能能力恢复的影响

Effect of rehabilitation of abnormal trunk posture on the recovery of exercise and functional ability in patients with hemiplegia

目的:研究躯干异常姿势康复对于脑卒中患者运动功能、日常生活活动能力的影响,并探讨其在脑卒中康复评估中的重要性和在脑卒中康复训练中的意义。方法:选取40例符合实验条件、于2017年11月至2018年10月在包头市中心医院进行康复治疗的偏瘫患者,随机分为实验组与对照组。治疗组在对照组常规康复治疗基础上加用躯干姿势调整,每天1次,每次40 min,每周5 d。观察比较两组相关量表评分及脊柱冠状面及矢状面形态参数。结果:(1)治疗前,两组TIS评分、Berg评分、Fugl-Meyer评分、Barthel指数评分比较差异均无统计学意义(P>0.05);治疗后,两组TIS评分、BBS评分、Fugl-Meyer评分、Barthel指数评分均显著高于治疗前(P<0.05),且治疗组均显著高于对照组。(2)治疗前后比较发现,除组内对比发现治疗后的脊柱正位片cobb角角度较治疗前有所改善(P<0.05)外,其余脊柱正侧位相关角度对比后均无统计学意义。结论:常规康复治疗联合躯干异常姿势康复治疗在改善患者运动及功能方面优于单纯常规康复治疗。

Objective:To study the effect of abnormal trunk posture rehabilitation on motor function and activities of daily living(ADL)in patients with stroke,and to explore its significance in stroke rehabilitation training.Methods:40 hemiplegic patients who met the experimental conditions and received rehabilitation treatment in Baotou Central Hospital from November 2017 to October 2018 were randomly divided into experimental group and control group.The treatment group was treated with trunk posture adjustment on the basis of routine rehabilitation treatment in the control group,once a day,40 minutes each time,and 5 days a week.The related scales and the coronal and sagittal morphological parameters of the spine were evaluated and compared in the two groups.Results:1.Before treatment,there was no significant difference in TIS score,Berg score,Fugl-Meyer score and Barthel index score between the two groups(P>0.05),but after treatment,TIS score,BBS score,Fugl-Meyer score and Barthel index score in the two groups were significantly higher than those before treatment(P<0.05),and those in the treatment group were significantly higher than those in the control group.2.It was found that in the intra-group comparison,the cobb angle of spinal positive position film was improved after treatment(P<0.05),but the other related angles of anterior and lateral position of spine had no statistical significance after treatment.Conclusion:Routine rehabilitation combined with trunk abnormal posture rehabilitation is superior to simple routine rehabilitation in improving the movement and function of patients.
