摘要
目的临床肺部感染评分(CPIS)对重症肺部细菌感染青年患者应用抗生素的指导效果,为其临床应用提供参考依据.方法选择86例确诊为重症肺部细菌感染的青年患者作为研究对象,采用随机数字法平均分为CPIS组与对照组,其中CPIS组治疗过程中以CPIS评分作为参考依据指导抗生素使用,对照组根据临床经验使用抗生素.比较两组抗生素使用时间、使用频度(DDDs)、住院时间、感染指标、死亡率等.结果CPIS组患者抗生素使用时间、DDDs均明显低于对照组(P<0.05);两组抗生素使用种类比较差异无统计学意义(P>0.05).两组患者随着入院时间的延长,PCT及CRP水平均明显降低(P<0.05);不同时间段两组患者PCT及CRP水平比较均无统计学意义(P>0.05).CPIS组患者住院时间及住院费用均明显低于对照组(P<0.05);CPIS组患者住院期间死亡率低于对照组,但差异无统计学意义(P>0.05).结论CPIS评分指导重症肺部细菌感染青年患者的抗生素使用可以明显降低抗生素使用时间、住院时间、住院费用及DDDs.
Objective The clinical pulmonary infection score(CPIS)guides the application of antibiotics in young patients with severe pulmonary bacterial infections,and provides a reference for its clinical application.Methods A total of 86 young patients diagnosed with severe pulmonary bacterial infection were selected as research objects.They were equally divided into the CPIS group and the control group by random number method.The CPIS group used the CPIS score as a reference guide to guide antibiotic use during treatment.Experience with antibiotics.The antibiotic use time,frequency of use(DDDs),length of hospital stay,infection index,mortality,etc.were compared between the two groups.Results The antibiotic use time and DDDs of CPIS group patients were significantly lower than those of the control group(P<0.05);there was no significant difference in antibiotic use types between the two groups(P>0.05).With the length of admission,the levels of PCT and CRP in the two groups decreased significantly(P<0.05);there was no significant difference in PCT and CRP levels between the two groups in different time periods(P>0.05).The hospitalization time and hospitalization costs of patients in the CPIS group were significantly lower than those in the control group(P<0.05);the mortality rate of patients in the CPIS group was lower than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion The CPIS score guides the use of antibiotics in young patients with severe pulmonary bacterial infections,which can significantly reduce antibiotic use time,hospital stay,hospitalization costs,and DDDs.
