摘要
目的了解深圳东部地区无偿献血人群中人类嗜T淋巴细胞病毒(HTLV)感染的流行情况。方法采用酶联免疫吸附试验(ELISA)对43 305名无偿献血者的标本进行HTLV-Ⅰ/Ⅱ抗体筛查,对其中的阳性的标本使用电化学发光(ECL)检测,检测阳性的标本再经RT-PCR或WB法确证并通过巢式PCR扩增进行基因测序和同源性分析。结果 ELISA法筛查HTLV-Ⅰ/Ⅱ抗体阳性的标本有33例,阳性率为0.076%;ECL检测阳性的标本5例,阳性率为15.15%;经RT-PCR或WB法确证阳性2例,确证感染率为0.004 6%;此2例病毒株均为HTLV-1A型,均为世界人种亚群。结论深圳东部地区无偿献血者属于HTLV低流行人群;是否将HTLV纳入无偿献血者常规筛查,有待进一步的监测数据支持。
Objective To evaluate the prevalence of antibodies to human T lymphocyte virus(HTLV)-Ⅰ/Ⅱ among voluntary blood donors in eastern Shenzhen area, China. Methods Initial screening for HTLV antibodies in 43 305 voluntary blood donors from eastern Shenzhen area were performed using enzyme-linked immunosorbent assay(ELISA). The reactive samples were re-tested by electroluminescence(ECL). The samples with repeated reactivity were confirmed by RT-PCR or Western blot(WB), and the gene sequencing and homology analysis were also performed by nested PCR amplification.Results Of 43 305 samples, 33 were initially reactive by ELISA(33/43 305, 0.076%), 5 were repeatedly reactive by ECL(5/33, 15.15%). Only 2 of 5 fulfilled the criteria for HTLV-1 positivity using RT-PCR or WB,and the rate of HTLV-1 infecition in blood donors of eastern Shenzhen was 0.004 6%(2//43 305).The two virus strains were both HTLV-1 genotype A subtype, and both were transcontinental subgroups. Conclusion Our results show that eastern Shenzhen is a HTLV-1 low endemic area. This preliminary data warrant further studies on the implementation of selective anti-HTLV-Ⅰ/Ⅱ screening of blood donors in this region.
