银屑病血瘀证患者外周血辅助性T细胞表达差异的研究

Study on differential expression of T helper cells in psoriasis with syndrome of blood stasis

目的观察银屑病血瘀证患者外周血辅助性T细胞(Th)表达的差异。方法选择银屑病血瘀证患者30例为银屑病组。同时选取30例健康志愿者作为对照组。采用酶联免疫吸附测定(ELISA)法分别检测2组外周血γ-干扰素(IFN-γ)、白细胞介素-4(IL-4)、白细胞介素-17A(IL-17A)、白细胞介素-10(IL-10)、白细胞介素-22(IL-22)水平,采用银屑病皮损面积和严重程度指数(PASI)评分评价银屑病组病情严重程度。对2组IFN-γ,IL-4,IL-17A,IL-10,IL-22水平进行比较,观察银屑病组细胞因子与PASI评分之间的相关性。结果银屑病组IFN-γ,IL-17A较对照组增高,差异均有统计学意义(P<0.05)。IL-4,IL-10,IL-22与对照组比较差异均无统计学意义(P>0.05)。银屑病组IL-17A/IL-10比值与PASI评分呈正相关(r=0.455,P<0.05),IFN-γ,IL-4,IL-17A,IL-10,IL-22以及IFN-γ/IL-4比值与PASI评分均无相关性(P>0.05)。结论 Th1,Th17可能在血瘀证中处于优势应答状态,Th2、调节性T细胞(Treg)、Th22可能无异常表达。Th17/Treg漂移可能在血瘀证中占据主导地位。

Objective To observe the different expression of T helper cells(Th) in serum of psoriasis with syndrome of blood stasis. Methods Levels of interferon-gamma(INF-γ), interleukin-4( IL-4),interleukin-17 A( IL-17 A),interleukin-10( IL-10) and interleukin-22( IL-22) in serum of 30 patients with psoriasis with syndrome of blood stasis and 30 healthy controls were detected by enzyme-linked immuno sorbent assay(ELISA). Psoriasis area severity index(PASI) scores were calculated to assess the severity.Relevances were analysed with the cytokines and the PASI scores. Results IFN-γ and IL-17 A were significantly higher in 30 patients than in healthy controls(P<0.05).The differences of IL-4,IL-10 and IL-22 between the two groups were not statistically significant(P>0.05).IL-17/IL-10 was positively related to the PASI scores significantly(r=0.455,P<0.05).There was no correlation between serum IFN-γ,IL-4,IL-17 A,IL-10,IL-22,IFN-γ/IL-4 and PASI scores(P>0.05). Conclusions Th1 and Th17 may be in the dominant response state in psoriasis with syndrome of blood stasis. While Th2,T regulatory cell(Treg) and Th22 may not have abnormal expression.Th17/Treg drift may play a dominant role in psoriasis with syndrome of blood stasis.
