老年高血压患合并心功能不全者可溶性ST2与左心室重塑的关系

The Relationship between Soluble ST2 and Left Ventricular Remodeling in Elderly Patients with Hypertension and Cardiac Insufficiency

目的探讨老年高血压合并慢性心功能不全患者sST2与其左心室重塑的关系。方法选择沈阳医学院附属第二医院2018—2019年收治的老年原发性高血压患者235例,依据是否存在心功能不全分为单纯高血压组(单纯组)146例和高血压伴心功能不全组(伴发组)89例;并且根据心功能进一步将伴发组分为轻度组57例和重度组32例,比较不同组间室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、左室舒张末期内径(LVEDD)、左室质量指数(LVMI)和ST2水平;并对伴发组LVMI与ST2进行相关性分析。结果单纯组IVST、LVPWT、LVMI与ST2均低于伴发组,差异有统计学意义(P<0.05)。轻度组IVST、LVPWT、LVMI与ST2均低于重度组,差异有统计学意义(P<0.05)。伴发组LVMI与ST2呈正相关关系(P<0.05)。结论老年高血压合并心功能不全患者ST2水平与左心室重塑存在一定关系,可作为一种生物学指标用于临床预测。

Objective To investigate the relationship between left ventricular remodeling and sST2 in elderly patients with hypertension and chronic cardiac insufficiency.Methods A total of 235 elderly patients with essential hypertension admitted to The Second Affiliated Hospital of Shenyang Medical College from 2018 to 2019 were divided into simple hypertension group(simple group)of 146 cases and hypertension with cardiac insufficiency group(concomitant group)of 89 cases according to the presence of cardiac dysfunction.Moreover,according to the cardiac function,the concomitant group was further divided into mild group of 57 cases and severe group of 32 cases,and the interventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricular end diastolic diameter(LVEDD),left ventricular mass index(LVMI)and ST2 were compared among different groups,and the correlation between LVMI and ST2 was analyzed in concomitant group.Results IVST,LVPWT,LVMI and ST2 in the simple group were significantly lower than those in the concomitant group(P<0.05).IVST,LVPWT,LVMI and ST2 in mild group were significantlylower than those in severe group(P<0.05).There was a positive correlation between LVMI and ST2(P<0.05).Conclusion There is a certain relationship between ST2 level and left ventricular remodeling in elderly patients with hypertension and cardiac insufficiency,which can be used as a biological index for clinical prediction.
