基于古今医案云平台的中药治疗成人咳嗽变异性哮喘用药规律分析

Analysis on Medication Rules of Chinese Medicine in Treating Adult Cough Variant Asthma Based on the Ancient and Modern Consilia Cloud Platform

[目的]通过“古今医案云平台(V2.1)”软件分析现代中医药治疗咳嗽变异性哮喘(cough variant asthma,CVA)的用药规律。[方法]运用回顾性研究方法,收集建库至2019年11月9日被中国期刊全文数据库(China National Knowledge Infrastructure,CNKI)、万方数据知识服务平台、维普中文科技期刊数据库收录的中医药治疗CVA的相关文献,严格按照纳入标准及排除标准,通过Note Express文献管理器,建立符合CVA的数据库,从而建立Excel数据表,将相关数据导入古今医案云平台软件,进行频次分析、中药属性分析、关联规则分析、聚类分析等数据挖掘,分析治疗CVA的用药规律。[结果]纳入文献196篇,录入196首方剂,合计188味中药。频次从高到低排名前10的中药单药分别为麻黄、苦杏仁、蝉蜕、地龙、桔梗、半夏、五味子、紫苏子、百部、僵蚕,药性以温居多,药味以辛为首,归经以肺、脾、胃、肝为主。演化得出麻黄-苦杏仁,麻黄-蝉蜕,苦杏仁-蝉蜕,麻黄、苦杏仁-地龙,蝉蜕、地龙-麻黄等核心组合18个;通过系统聚类分析推演出治疗CVA的新处方共计4个。[结论]现代中医药治疗成人CVA多从肺、脾、肝三脏出发,从辛温理气入手,注重宣肺、理气、化痰、祛风,尤其重视虫类祛风通络药的应用,体现了宣肺祛痰止咳、降逆利气、解痉平喘的治疗原则。

[Objective]To analyze the medication rules of modern Chinese medicine in the treatment of cough variant asthma(CVA)through the Ancient and Modern Consilia Cloud Platform(V2.1),and provide a reference for clinical treatment of the disease.[Methods]Using retrospective study method,the related literature of CVA treated by traditional Chinese medicine(TCM)published in China national knowledge infrastructure(CNKI),Wanfang Data Knowledge Service Platform,China Science and Technology Journal Database from beginning to November 9,2019 were collected.In strict accordance with the inclusion criteria and exclusion criteria,using the Note Express document manager,a database of CVA was set up,thus an Excel data table was established,and the relevant data were input into Ancient and Modern Consilia Cloud Platform(V2.1),then data mining such as frequency analysis,attribute analysis of TCM,association rules analysis and cluster analysis were carried out through the software to analyze the medication rules in the treatment of CVA,so as to provide certain idea for the clinical medication.[Results]The total of 196 literatures were included,by strict accordance with the inclusion criteria and screening standard,a total of 196 prescriptions and 188 kinds of medicinal herbs were screened out and input.According to the systematic analysis,the top 10 medicinal herbs use frequency from high to low were Ephedra sinica,Armeniacae Semen,Periostracum cicada,Lumbricus,Platycodon grandiflorus,Pinellia ternata,Schisandra chinensis,Fructus perillae,Radix stemonae,Bombyx batryticatus.Most medicinal herbs have pungent taste,warm property and the channel tropism were the lung,spleen,stomach and liver.Eighteen core combinations of Ephedra sinica-Armeniacae Semen,Ephedra sinica-Periostracum cicada,Armeniacae Semen-Periostracum cicada,Ephedra sinica and Armeniacae Semen-Lumbricus.and 4 new prescriptions for the treatment of CVA were deduced through systematic cluster analysis,and a new prescription 1(Ephedra sinica,Armeniacae Semen,Periostracum cicada,Lumbricus)and so on.[Conclusion]TCM treatment of adult CVA is mainly based on lung,spleen and liver,starts from regulating Qi with pungent and warm drugs,focuses on promoting the dispersing function of the lung,regulating Qi,reducing phlegm and dispelling wind,especially the application of insects drugs for dispelling wind and dredging collaterals;the therapeutic principles of relieving cough,relieving spasm and relieving asthma are presented.
