电针结合郑氏三号熏洗药治疗慢性跖筋膜炎合并功能性腓肠肌挛缩随机对照研究

A Randomized Controlled Clinical Trial on Treatment of Chronic Plantar Fasciitis with Functional Gastrocnemius Contracture with Electro-acupuncture combined with Zheng’s Fumigation and Washing Medicine Ⅲ

目的:研究电针结合郑氏三号熏洗药治疗慢性跖筋膜炎合并功能性腓肠肌挛缩的临床疗效。方法:选取70例慢性跖筋膜炎合并功能性腓肠肌挛缩患者为研究对象,按随机数字表法,随机分为治疗组和对照组,各35例,对照组每日进行常规的跖筋膜牵张训练及跟腱牵张训练,治疗组在对照组基础上进行电针联合郑氏三号熏洗药治疗,共治疗4周。治疗前、治疗后2周及4周进行视觉模拟量表(VAS)评分、美国足踝外科协会踝-后足功能量表(AOFAS-AHS)评分。结果:治疗后2、4周两组患者VAS评分均较治疗前降低,且治疗组低于对照组(P<0.05);治疗后2、4周两组患者AOFAS-AHS评分均较治疗前升高,且治疗组高于对照组(P<0.05)。结论:电针结合郑氏三号熏洗药治疗慢性跖筋膜炎合并功能性腓肠肌挛缩具有良好临床疗效,优于单纯牵张训练。

Objective: To study the efficacy of electro-acupuncture combined with Zheng’s fumigation and washing medicine Ⅲ(ZSFWM Ⅲ) in the treatment of chronic plantar fasciitis with gastrocnemius contracture.Methods: 70 patients with chronic plantar fasciitis combined with functional gastrocnemius contracture were randomly divided into two group: control group(n=35) and treatment group(n=35). The control group was treated with plantar fasciitis stretching training and achilles tendon stretching training. The treatment group was treated with ZSFWM Ⅲ combined with electro-acupuncture on basis of the control group, for 4 weeks. Visual analogue scale(VAS) scores and American orthopaedic foot and ankle society-ankle hindfoot scale(AOFAS-AHS) scores were obtained before treatment, two weeks after treatment, and four weeks after the end of the course of the treatment. Results: The VAS scores of the two groups were decreased at 2 and 4 weeks after treatment, and the treatment group was lower than the control group(P<0.05);the AOFAS-AHS scores of the two groups were increased at 2 and 4 weeks after treatment, and the treatment group was higher than the control group(P<0.05).Conclusion: Electro-acupuncture combined with ZSFWM Ⅲ has a good clinical efficacy in the treatment of plantar fasciitis with functional contracture of gastrocnemius, which is superior to stretch training.
