健康宣教联合MOTOmed下肢运动训练在脑卒中偏瘫患者中的应用

Application of health education combined with MOTOmed exercise training of lower limbs in stroke patients with hemiplegia

目的探讨健康宣教联合MOTOmed下肢运动训练在脑卒中偏瘫患者中的应用效果。方法选取2017年7月至2019年6月我院康复科收治的104例脑卒中偏瘫患者,根据随机数字表法等分为观察组和对照组,对照组给予常规康复训练结合健康宣教,观察组在对照组基础上加用MOTOmed下肢运动训练。干预3个月,观察两组患者下肢肌力、平衡功能、下肢运动功能及步行、步态功能、日常生活活动能力和依从性。结果干预后,观察组Lovett分级法、Berg平衡量表、简式Fugl-Meyer运动功能量表、Barthel指数得分均高于对照组,差异具有统计学意义(P<0.05)。观察组功能性步行分级(FAC)≥3级患者多于对照组,最大步行速度、步长、步频、Tinetti步态评测表得分均优于对照组(P<0.05),Ashworth评分量表得分低于对照组(P<0.05)。观察组依从性优良率明显高于对照组(P<0.05)。结论健康宣教联合MOTOmed下肢运动训练有利于改善脑卒中偏瘫患者下肢肌力、平衡功能、下肢运动功能,步行、步态功能,提高依从性及生活质量。

Objective To explore the application effect of health education combined with MOTOmed exercise training of lower limbs in stroke patients with hemiplegia.Methods To select 104 cases of stroke patients with hemiplegia admitted to the rehabilitation department of our hospital from July 2017 to June 2019,and they were divided into an observation group and a control group according to the random number table method.The control group was given regular rehabilitation training combined with health education,the observation group was given MOTOmed exercise training of lower limbs on the basis of the control group.After intervention for 3 months,the muscle strength of the lower limbs,balance function,motor function and walking of lower limbs,gait function,activities of daily living and compliance of patients were observed between the two groups.Results After the intervention,the scores of Lovett classification,Berg balance scale,short-form Fugl-Meyer motor functional scale,and Barthel index score of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The observation group had more patients with functional ambulation category(FAC)≥3 than those the control group,and the maximum walking speed,step length,step frequency,and Tinetti gait evaluation table scores were better than those of the control group(P<0.05),and the Ashworth scale score was lower than that in the control group(P<0.05).The good rate of compliance in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Health education combined with MOTOmed exercise training of lower limbs was beneficial to improve muscle strength of lower limbs,balance function,motor function of lower limbs,walking and gait function,and improve compliance and living quality of stroke patients with hemiplegia.
