摘要
目的探讨血清Ⅱ型肺泡上皮细胞表面抗原6(KL-6)及肺泡表面活性蛋白D(SP-D)水平与肺功能检查对结缔组织性肺间质病变的临床价值。方法选取2018年1月至2019年10月在本院住院治疗的结缔组织性肺间质病变患者82例为实验组,另选取同期于本院行健康体检健康者30名作为对照组。比较两组血清SP-D、KL-6水平、肺功能,并分析实验组中SP-D、KL-6水平与肺功能指标相关性。结果实验组SP-D、KL-6水平为(753.01±212.29)g/L、(252.89±65.24)kU/L,高于对照组的(67.56±17.59)g/L、(132.30±42.98)kU/L,差异有统计学意义(P<0.05);实验组FVC、TLC、FEV1水平为(65.42±12.49)L、(79.52±6.88)L、(75.62±9.97)L,低于对照组的(94.60±1.41)L、(98.12±0.98)L、(92.59±1.30)L,差异有统计学意义(P<0.05);SP-D、KL-6水平与FVC、TLC、FEV1呈负相关(P<0.05)。结论血清SP-D、KL-6和肺功能指标均是检测结缔组织性肺间质病变的有效指标,SP-D、KL-6与肺功能指标呈负相关,能为病情判断和预后评估提供帮助,指导临床治疗,并改善预后。
Objective To investigate the clinical value of serum krebs von den lungen 6(KL-6)and surfactant protein D(SP-D)and pulmonary function examination in interstitial lung disease of connective tissue disease.Methods 82 patients with interstitial lung disease of connective tissue disease who hospitalized in the hospital from January 2018 to October 2019 were selected as the experimental group,and 30 healthy people who underwent physical examination during the same period were selected as the control group.The serum SP-D,KL-6,pulmonary function were compared between the two groups,and the correlation of SP-D and KL-6 levels with pulmonary function indexes in the experimental group was analyzed.Results The levels of SP-D and KL-6 in the experimental group[(753.01±212.29)g/L,(252.89±65.24)kU/L]were higher than those in the control group[(67.56±17.59)g/L,(132.30±42.98)kU/L],with statistically significant difference(P<0.05).The levels of FVC,TLC,and FEV1 in the experimental group[(65.42±12.49)L,(79.52±6.88)L,(75.62±9.97)L]were lower than those in the control group[(94.60±1.41)L,(98.12±0.98)L,(92.59±1.30)L],with statistically significant difference(P<0.05).The levels of SP-D and KL-6 were negatively correlated with FVC,TLC and FEV1(P<0.05).Conclusion Serum SP-D,KL-6 and pulmonary function indexes are all effective indicators for detecting interstitial lung disease of connective tissue disease.SP-D and KL-6 are negatively correlated with pulmonary function indexes,which can provide help for disease judgment and prognostic assessment to guide clinical treatment and improve prognosis.
