摘要
目的探讨胱抑素C(CysC)、尿微量白蛋白(mAlb)及β2-微球蛋白(β2-MG)检测对妊高症早期肾损伤的诊断价值。方法选取2017年5月至2019年7月就诊于本院的120例妊高症患者,按是否合并早期肾损伤分为合并肾损伤组(n=45)与未合并肾损伤组(n=75),选择同期在本院例行产检的50名正常妊娠孕妇为对照组。采集受检者空腹静脉血或晨尿,检测CysC、β2-MG、尿mAlb。分析3组尿mAlb、CysC、β2-MG水平,各指标单一与联合检测价值。结果合并肾损伤组mAlb、CysC、β2-MG水平均高于未合并肾损伤组、对照组(P<0.05),未合并肾损伤组mAlb、CysC、β2-MG水平均高于对照组(P<0.05)。尿mAlb、CysC、β2-MG联合检测的准确度和敏感度高于尿单一检测(P<0.05);尿mAlb、CysC、β2-MG联合检测特异度高于尿mAlb、β2-MG单一检测(P<0.05)。结论尿mAlb、CysC、β2-MG均是检测妊高症早期肾损伤的有效指标,联合检测可提高检测准确度和敏感度,指导临床治疗,改善患者预后。
Objective To investigate the diagnostic value of Cystatin C(CysC),urinary microalbumin(mAlb)andβ2-microglobulin(β2-MG)in early renal injury of pregnancy induced hypertension.Methods 120 patients with pregnancy-induced hypertension who were admitted to our hospital from May 2017 to July 2019 were enrolled.They were divided into the renal injury group(n=45)and without renal injury group(n=75)according to whether they had early renal injury.50 normal pregnant women who were routinely examined in our hospital during the same period were selected as the control group.The subjects were collected for fasting venous blood or morning urine,and the level of CysC,β2-MG,urine mAlb were compared among the three groups.The urinary mAlb,CysC,β2-MG single and combined detection values were analyzed.Results The levels of mAlb,CysC andβ2-MG in the renal injury group were significantly higher than those without renal injury and the control group(P<0.05);the accuracy and sensitivity of the combined detection of urinary mAlb,CysC andβ2-MG were significantly higher than the single urine test(P<0.05);the specificity of urine mAlb CysC andβ2-MG combined detection was significantly higher than that of mAlb CysC andβ2-MG single test(P<0.05).Conclusion Urine mAlb,CysC andβ2-MG are effective indicators for detecting early renal injury in pregnancy-induced hypertension.Combined detection can improve detection accuracy and sensitivity,guide clinical treatment and improve patient prognosis.
