摘要
目的比较超声引导下水压灌肠与X线监视下空气灌肠对小儿肠套叠的疗效。方法回顾性分析2017年1月至2020年1月郑州大学第一附属医院收治的432例肠套叠患儿的临床资料,根据灌肠方法分为对照组(192例)和观察组(240例)。给予对照组X线监视下空气灌肠复位,给予观察组超声引导下水压灌肠复位。比较两组复位成功率、灌肠前入院时间、灌肠用时、C反应蛋白(CRP)、复套发生率以及并发症发生率。结果观察组复位成功率[92.1%(221/240)]高于对照组[82.3%(158/192)],灌肠前入院时间和灌肠用时均短于对照组,灌肠12 h后CRP低于对照组(均P<0.05)。两组复套发生率和并发症发生率比较,差异无统计学意义(均P>0.05)。结论超声引导下水压灌肠复位方便易行,复位率高,安全有效,无辐射,易被医患双方接受,值得进一步推广应用。
Objective To compare the therapeutic effect of ultrasound guided hydrostatic enema and air enema treatment under X-ray guide on children intussusception.Methods The clinical data of 432 children with intussusception admitted to the First Affiliated Hospital of Zhengzhou University from January 2017 to January 2020 were analyzed retrospectively.They were divided into control group(192 cases)and observation group(240 cases)according to the enema method.Control group was given air enema treatment under X-ray guide.Observation group was given by ultrasound guided hydrostatic enema.The success rate of reduction,admission time before enema,enema time,C reactive protein(CRP),recurrence rate and complication rate between the two groups were compared.Results The success rate of reduction[92.1%(221/240)]in observation group was higher than that[82.3%(158/192)]in control group,and the admission time before enema and enema time in observation group were both lower than those in control group,and the CRP in observation group was lower than that in control group after 12 hours of enema(all P<0.05).There were no statistical differences on recurrence rate and complication rate between the two groups(both P>0.05).Conclusion Ultrasound guided hydrostatic enema is a convenient and easy therapy method for children intussusception,with high reduction rate.It is safe and effective with no radiation and easy to be accepted by doctors and patients.It is worthy of further promotion and application.
