四维彩超检测胎儿心上方静脉发育异常与临床研究

Abnormal Development of Fetal upper Cardiac Vein Detected by Four-dimensional Color Ultrasound and its Clinical Study

目的分析四维超声HD-Flow显像及常规二维超声在胎儿心上方静脉发育异常的声像图特征,比较二者在产前诊断胎儿心上方静脉发育异常的临床价值。方法采用四维HD-flow技术和常规二维超声技术,对随机选取的1032例胎儿心上方大静脉进行血管重建成像,分析两种技术对可疑心上方静脉发育异常诊断的临床结果。结果 1032例胎儿在HD-flow技术下数据采集成功率约95%。常规二维超声心动图诊断上腔静脉发育异常26例;而四维HD-flow技术对其重新筛查发现:其中1例伴左上腔静脉缺如,2例右上腔静脉与左心房连接,3例部分型肺静脉异位引流,1例右侧上腔静脉走行异常合并下腔静脉离断,19例为单纯的永存左上腔静脉。结论四维HD-flow技术与常规二维超声技术比较,显示胎儿上腔静脉、肺静脉条数更有优势,有利于提高胎儿心上方静脉发育异常诊断的准确性,并且为临床治疗提供更可靠的依据。

Objective To analyze the sonographic features of four-dimensional color ultrasound HD-flow imaging and conventional 2-D ultrasound in fetal upper heart vein dysplasia,and to compare their clinical value in prenatal diagnosis of fetal upper heart vein dysplasia.Methods Using four-dimensional HD-flow technology and conventional two-dimensional ultrasound technology,vascularization imaging was performed on fetal superior cardiac veins of randomly selected 1032 fetuses.Results The success rate of data acquisition in 1032 fetuses with HD-flow technique was about 95%.Routine twodimensional echocardiography was used to diagnose 26 cases of abnormal development of superior vena cava.Four-dimensional HD-flow technique was used to re-screen 26 cases of abnormal development of superior vena cava.Among them,1 case had absence of left superior vena cava,2 cases had right superior vena cava connected with left atrium,3 cases had partial anomalous pulmonary venous drainage,1 case had abnormal course of right superior vena cava with inferior vena disconnection,19 Permanent left superior vena cava.Conclusion Intuitively display their spatial position relationship,which is conducive to improving the accuracy of diagnosis of fetal superior cardiac venous dysplasia and providing more reliable basis for clinical treatment.
