甲状腺癌超声征象与病理组织学类型及临床分期的相关性

Correlation of Ultrasonic Features with Histopathological Types and Clinical Stages of Thyroid Carcinoma

目的探讨甲状腺癌超声征象与病理组织学类型及临床分期的相关性。方法选取2015年10月至2018年12月在辽宁中医药大学附属医院治疗的200例甲状腺癌患者,均行彩色超声诊断,对相关资料进行回顾性分析。所有患者均经病理诊断确诊,分析甲状腺癌超声征象与病理组织学类型及临床分期的相关性。结果经超声诊断发现,阳性患者195例,误诊及漏诊5例,超声诊断准确率为97.50%。超声诊断结果与病理诊断结果比较,差异无统计学意义(P> 0.05)。在确诊的195例患者中,滤泡状癌17例,甲状腺乳头状癌159例,髓样癌6例,未分化癌13例。195例甲状腺乳腺癌按照TNM分期标准分级:Ⅰ期70例,Ⅱ期83例,Ⅲ期15例,Ⅳ期27例。经单因素分析发现,在甲状腺癌超声征象中,边缘不规则、纵横比≥1及微小钙化灶癌灶的病理组织变化之间密切相关。结论甲状腺癌的超声征象与病理组织学类型之间存在一定的相关性,通过观察甲状腺癌超声征象有助于鉴别肿块的病理类型,可为临床诊断和治疗提供可靠的参考。

Objective To investigate the correlation between ultrasonographic features and histopathological types and clinical stages of thyroid carcinoma.Methods From October 2015 to December 2018,200 cases of thyroid cancer patients in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected,and the relevant data were retrospectively analyzed.All patients were confi rmed by pathological diagnosis.The correlation between ultrasonic signs of thyroid carcinoma and histopathological type and clinical stage was analyzed.Results Among of them,195 cases were positive and 5 cases were misdiagnosed and missed.The accuracy rate of ultrasound diagnosis was 97.50%.There was no signifi cant diff erence between ultrasonic diagnosis and pathological diagnosis(P>0.05).Among 195 patients,17 were follicular carcinoma,159 papillary thyroid carcinoma,6 medullary carcinoma and 13 undiff erentiated carcinomas.According to TNM staging standard,195 cases of thyroid breast cancer were classifi ed as stageⅠ70 cases,stageⅡ83 cases,stageⅢ15 cases,stageⅣ27 cases.Univariate analysis showed that there was a close relationship between irregular margin,aspect ratio≥1 and pathological changes of microcalcifi cations.Conclusion There is a certain correlation between ultrasonic signs and histopathological types of thyroid carcinoma.The observation of ultrasonic signs of thyroid cancer is helpful to diff erentiate the pathological types of the tumor,and can provide reliable reference for clinical diagnosis and treatment.
