河南省51家三级公立医院病案编码员现状分析 被引量:7

Analysis on the Current Situation of Medical Record Coders in 51 Tertiary Public Hospitals in Henan Province

摘要 目的分析河南省内三级公立医院病案编码员现状,为加强医院病案队伍管理,提升其综合能力提供参考依据。方法以2019年9月河南省三级公立医院编码员为研究对象,采用问卷调查法,调查内容包括病案编码人员的学历构成、专业背景、职称及年工作量4大类23项,共获得全部样本211份,采用SPSS21.0进行数据分析。结果三级公立医院编码员中具有本科学历者占主体,具有大学本科及以上学历者占总体83.8%,整体学历水平尚可,但具有医学专业背景者仅有48.8%,略显不足;编码员职称普遍较低,其中具有高级职称者仅5.7%,且缺少专门匹配该岗位的高级职称;编码员需要承担的平均工作量较为繁重,但省、市2级的综合性医院与专科医院之间工作量均具有显著差异(t=2.127,P=0.048;t=2.085,P=0.045)。结论三级公立医院编码员整体素养较好,但存在专业背景不符、职称评定不畅、工作量较大等问题,医院及管理部门应引进专业人才,减轻编码员工作量,填补病案高级职称空白,加强该群体建设。 Objectives We analyzed the status of medical record coders in 51 tertiary public hospitals in Henan Province,so as to provide reference for strengthening the management of medical record teams in hospitals and improving their comprehensive capabilities.Methods Taking the coder of the tertiary public hospital of Henan Province in September 2019 as the research object,the questionnaire survey method was adopted.The survey content included the academic background,professional background,professional title and annual workload of the coder of the medical record,23 items in 4 categories,and 211 samples were obtained in total.SPSS21.0 was used for data analysis.Results The number of coders with bachelor’s degree was the main body of the tertiary public hospitals in the province,83.8%of them had bachelor’s degree or above,and the overall education level was acceptable,but only 48.8%of them had medical professional background,which was obviously insufficient,the general title of coders was relatively low,only 5.7%of them had senior title,and there was no senior title specially matching the post,coders needed to undertake the average workload was heavy,but there were significant differences between provincial and municipal general hospitals and specialized hospitals(t=2.127,P=0.048;t=2.085,P=0.045).Conclusions The overall quality of the coders in the tertiary public hospitals was good,but there were some problems such as inconsistent professional background,poor professional title evaluation and heavy workload.Hospitals and management departments should introduce professional talents,reduce the workload of coders,fill in the gap of senior medical record titles,and strengthen the construction of the group.
