重症超声导向的新型冠状病毒肺炎管理策略 被引量:4

The management strategy of coronavirus disease 2019: what can critical ultrasound do?

摘要 新型冠状病毒肺炎(COVID-19)主要的靶器官为肺部,导致肺部渗出、实变,并可累及心血管、凝血等多个系统。在疾病进展过程中,可能出现因呼吸驱动过度增强加重肺部损伤,甚至出现气胸;部分患者因血栓形成导致肺栓塞;这些并发症均会出现严重的低氧血症,进一步加重肺及其他器官功能的损害。重症超声以目标为导向,通过对心、肺及其他器官的流程化评估,快速筛查低氧血症原因。此外,COVID-19患者出现"沉默型低氧血症",导致不能在早期及时发现重症患者的呼吸代偿程度。重症超声通过对膈肌活动的评估,可早期发现呼吸驱动增强,为早期发现重症患者提供可视化诊疗依据,并在诊治过程中动态评估患者病情。
