2型糖尿病合并尿路感染的病原菌分布及实验室结果 被引量:6

摘要 目的分析并探讨2型糖尿病(T2DM)合并尿路感染(URI)的人群特点、病原菌分布及实验室检查指标特征。方法选取2017年2月-2019年2月期间信阳市中心医院收治的476例T2DM合并URI患者和500例单纯T2DM患者进行前瞻性研究。分析T2DM合并URI组患者人群特点、病原菌分布情况、主要致病菌耐药性特点,比较两组实验室生化检查结果。结果 T2DM合并URI患者中,女性患者分布率高于男性患者,年龄≥60岁患者分布率高于年龄<60岁患者,临床表现中以无症状菌尿分布率最高;476例T2DM合并URI患者分离出的病原菌中以大肠埃希菌为主(占53.97%),其对亚胺培南耐药率最低;与单纯T2DM组相比,T2DM合并URI组甘油三酯(TG)水平显著较高(P<0.05)。结论 T2DM合并URI以女性患者、年龄≥60岁患者为主,临床多表现为无症状菌尿,主要致病菌是大肠埃希菌,高水平TG可能与T2DM合并URI的发生相关。 Objective To analyze and investigate the group characteristics,pathogens distribution and laboratory test indexes of type 2 diabetes mellitus(T2DM) with urinary tract infection(URI).Methods 476 cases of T2DM patients with URI and 500 patients with simple T2DM admitted to our hospital from February 2017 to February 2019 were selected for prospective study.The group characteristics,the pathogens distribution and drug resistance of major pathogens were analyzed in the T2DM with URI group,and the results of laboratory biochemical tests were compared between the two groups.Results Among patients with T2DM combined with URI,the distribution rate of female patients was higher than that of male patients,and the distribution rate of patients aged≥60 years was higher than that of patients <60 years.Asymptomatic bacteriuria was the highest in clinical manifestations.Among the pathogens isolated from 476 patients with T2DM combined with URI,Escherichia coli(53.97%) was the main pathogen,and the resistance rate to imipenem was the lowest.The level of triglyceride(TG) of the T2DM combined with URI group was significantly higher than that of T2DM group(P<0.05).Conclusion Patients of T2DM combined with URI were mainly female and aged 60 years.The clinical manifestations were asymptomatic bacteriuria.The main pathogen was Escherichia coli.High levels of TG might be related to the occurrence of T2DM combined with URI.
