冷圈套器对4~15 mm无蒂息肉的治疗效果 被引量:9

摘要 目的在中国人群中对冷圈套器与传统高频电凝切除结直肠息肉的安全性进行比较,特别评估10~15 mm较大息肉冷圈套器切除的安全性。方法选择355例冷圈套器切除(cold snare polypectomy,CSP)息肉及245例高频电凝切除(hot snare polypectomy,HSP)息肉,治疗息肉直径4~15 mm,其中包括78例10~15 mm CSP治疗息肉及95例10~15 mm HSP治疗息肉。排除无蒂息肉。对CSP及HSP息肉的平均治疗时间、并发症发生情况、完整切除率、病变回收率等进行比较。结果全部CSP治疗息肉与HSP治疗息肉进行比较,CSP组即时出血率与HSP组差异有统计学意义,最终两组的迟发出血率均为0。CSP组平均治疗时间显著低于HSP组。两组穿孔并发症发生率、完整切除率、病变回收率差异无统计学意义。在10~15 mm病变组,CSP组平均治疗时间显著低于HSP组,两组病变出血、穿孔并发症发生率、完整切除率、病变回收率差异无统计学意义。结论CSP与HSP同样为4~15 mm无蒂息肉安全的治疗方式,CSP较HSP更为节省时间。 Objective To investigate the efficacy and safety of cold snare polypectomy(CSP)for complete resection of 4~15 mm colorectal polyps compared with that of hot snare polypectomy(HSP),especially assessed the efficacy and safety of CSP for 10-15 mm polyps.Methods355 cold snare polypectomy cases and 245 hot snare polypectomy cases were included,exclude pedunculate polyps.Mean treatment time,retrieval rates,complete resection rates,and complication rates were assessed and compared between CSP group and HSP group for polyps sized 4~15 mm and 10~15 mm.Results Immediate bleeding rate in CSP group is higher than HSP group for polyps sized 4~15 mm.Complete resection rates,retrieval rates and perforation rates were equal in CSP group and HSP group for polyps sized 4-15 mm.Complete resection rates,retrieval rates,perforation rates and bleeding rates were equal in CSP group and HSP group for polyps sized 10-15 mm.Mean treatment time of CSP is shorter than HSP.ConclusionCSP and HSP are both safety and effective in sessile polyps polypectomy.CSP save time than HSP.
