单根可吸收缝线交互式内翻缝合在肠肠全手工吻合中的应用

Application of single resorbable suture interactive inversion suture in full manual anastomosis

目的介绍应用单根可吸收缝线行肠肠全手工吻合的具体操作方法及应用价值。方法2017年7月~2018年6月因腹盆腔肿瘤性疾病行肠肠吻合病人224例,根据吻合方式不同分为两组,手工组98例,行单根可吸收缝线全手工肠肠吻合;器械组126例,行器械吻合加丝线加固。比较两组吻合时间、吻合口出血、吻合口狭窄、吻合费用等,统计分析单根可吸收缝线交互式内翻缝合在肠肠全手工吻合中的应用价值。结果224例病人均顺利完成手术,手工组与器械组术后吻合口渗血分别为3例和15例,吻合口出血分别为1例和19例,吻合口狭窄分别为0例和13例,手术时间分别为(3.2±1.1)分钟和(5.5±2.1)分钟,吻合费用分别为(53±13.2)元和(2753±899.2)元,两组比较差异有统计学意义(P<0.05)。结论应用单根可吸收缝线行交互式内翻缝合在全手工肠肠吻合中安全可靠,吻合相关并发症少,有利于减少手术时间,降低住院费用。

Objective To introduce the specific operation method and application value of applying a single absorbable suture for manual bowel anastomosis.Methods From July 2017 to June 2018,a total of 224 patients underwent manual bowel anastomosis due to abdominal and pelvic neoplastic diseases;they were divided into two groups according to different methods of anastomosis,98 cases in the manual group,with a single absorbable suture.126 patients in the instrument group,with silk thread reinforcement additionally.The anastomosis time,anastomotic hemorrhage,anastomotic stenosis,and anastomotic cost were compared between the two groups,and the application value of single absorbable suture interactive inversion suture in manual bowel anastomosis was statistically analyzed.Results All 224 patients were successfully completed the operation.The postoperative anastomotic leakage in the manual group and the instrument group were 3 and 15 respectively,the anastomotic bleeding was 1 and 19,and the anastomotic stenosis was 0 and 13 cases.The operation time was(3.2±1.1)min and(5.5±2.1)min,and the cost of anastomosis was(53±13.2)yuan and(2753±899.2)yuan respectively.The difference between the two groups was statistically significant.ConclusionUsing a single absorbable suture in interactive inversion suture is a safe and reliable method in manual bowel anastomosis,with fewer complications associated with anastomosis,which is beneficial to reduce the operation time and reduce the cost of hospitalization.
