基于数据挖掘从医案和验方探析曾庆琪教授辨治慢性前列腺炎的学术思想

An Analysis of Professor Zeng Qingqi’s Academic Thoughtson the Treatment of Chronic Prostatitis from Medical Cases and Prescriptions Based on Data Mining

目的探讨全国著名中医男科专家曾庆琪教授治疗慢性前列腺炎(CP)的学术经验,指导临床运用,传承名中医学术思想。方法检索中国知网(CNKI)、维普(VIP)、中国生物医学文献(CBM)等文献数据库,收集曾庆琪教授发表于期刊中涉及治疗CP的验方及医案;将药物及治疗病证录入中医传承辅助平台V2.5软件,记录并统计“用药频次”“组方规律”“四气”“五味”“归经”情况。结果检索数据库收集曾庆琪教授治疗CP的学术论文34篇,共获取曾庆琪教授治疗CP验方28首。用药位列前6位的是萆薢、茯苓、石菖蒲、菟丝子、乌药、甘草;位列前7位的药物组合分别是茯苓-萆薢;菟丝子-萆薢;茯苓-石菖蒲;萆薢-石菖蒲;菟丝子-石菖蒲;乌药-石菖蒲;菟丝子-萆薢-石菖蒲;“温”性药物使用频次最高,“平”性药物次之;“辛”味药物使用频次最高,“苦”味药物次之;归肾经的药使用频次最高,归肝经的药物次之。结论曾庆琪教授不断创新完善男科“精室理论”,认为CP的发生,体质虚弱是病变基础,充血感染是重要因素,基本病机是湿热下注、败精瘀浊、精室被扰、精关不固、封藏失职,以“洁净精室”为基本治则,辨证论治的思想主要体现在活用经方、分期论治、从肝论治、创立新方4个方面。

Objective To explore the academic experience of Professor Zeng Qingqi,a famous Chinese medicine andrology expert,in the treatment of chronic prostatitis,to guide the clinical application,and to inherit the academic thought of famous Chinese medicine.Methods We searched CNKI and other literature databases and collected the prescriptions and medical cases related to the treatment of chronic prostatitis published in the journal by Professor Zeng Qingqi.The medicines and treatment syndromes were recorded in the V2.5 software of the traditional Chinese medicine inheritance and assistance platform,and the frequency of drug use,rules of prescriptions,four Qi,five flavors,meridian return were recorded and counted.Results 34 academic papers about Professor Zeng Qingqi’s treatment of chronic prostatitis were collected from CNKI and other databases,and 28 prescriptions of Professor Zeng Qingqi’s treatment of chronic prostatitis were obtained.The top 6 medications are Diabetes,Poria,Acorus,Dodder,Dodder,and Licorice.The top 7 drug combinations are Poria-Abrax,Dodder-shichangpu,Poria-shichangpu,Brachy-Acorus,Cuscuta-shichangpu,Wuyao-shichangpu and Cuscuta-seeda-shichangpu.The warm drugs were used most frequently and then the flat drugs.The Xin drugs were used most frequently,and then the bitter drugs.The most frequently used drugs are the Kidney Meridian,followed by the Liver Meridian.Conclusion:Professor Zeng Qingqi constantly innovates and improves the“seminal chamber theory”of andrology.He believes that for the occurrence of chronic prostatitis,physical weakness is the basis of pathological changes and hyperemia and infection are important factors.The basic pathogenesis are damp heat injection,abortion,stasis and turbidity,disturbance of seminal chamber,unsteadiness of seminal shut-off,and failure of sealing and storing.Taking“seminal chamber clean”as the basic treatment principle,the thought of syndrome differentiation and treatment is mainly reflected in the“flexible use of classical prescriptions”.Four aspects are discussed:treatment by stages,treatment by liver,and creation of new.
