摘要
目的探讨合并抗Ro52抗体阳性的抗Jo-1抗体综合征的临床特点及生存分析。方法抗Jo-1抗体综合征患者84例,其中合并抗Ro52抗体阳性者41例为阳性组,抗Ro52抗体阴性者43例为阴性组。比较2组临床特征,采用多因素logistic回归分析抗Jo-1抗体综合征患者发生抗Ro52抗体阳性的危险因素,采用Kaplan-Meier法分析2组生存情况。结果阳性组患者病程[(15.9±13.2)个月]短于阴性组[(25.0±21.6)个月](P<0.05),阳性组患者关节炎、雷诺现象和Gottron征发病率(87.8%、58.5%、73.2%)高于阴性组(58.1%、27.9%、51.2%)(P<0.05);关节炎(OR=5.988,95%CI:1.805~19.869,P=0.003)、雷诺现象(OR=3.666,95%CI:1.076~12.482,P=0.038)是抗Jo-1抗体综合征出现抗Ro52抗体阳性的独立危险因素;阳性组中位生存时间(24个月)短于阴性组(25个月)(P<0.05)。结论合并抗Ro52抗体阳性的抗Jo-1抗体综合征患者病程短,关节炎和雷诺现象是发生抗Ro52抗体阳性的独立危险因素。
Objective To investigate the clinical characteristics and survival of anti-Jo-1 antibody syndrome patients with positive anti-Ro52 antibody. Methods Eighty-four patients with anti-Jo-1 antibody syndrome included 41 patients with positive anti-Ro52 antibody(positive group) and 43 patients with negative anti-Ro52 antibody(negative group). The clinical characteristics were compared between two groups. Multivariate logistic regression analysis was performed to analyze the risk factors for anti-Ro52 antibody in patients with anti-Jo-1 antibody syndrome. The survival was compared by Kaplan-Meier method between two groups. Results The course of disease was shorter in positive group((15.9±13.2) months) that that in negative group((25.0±21.6) months)(P<0.05), and the incidences of arthritis, Raynaud’s phenomenon and Gottron sign were higher in positive group(87.8%, 58.5%, 73.2%) than those in negative group(58.1%, 27.9%, 51.2%)(P<0.05). Arthritis(OR=5.988, 95%CI: 1.805-19.869, P=0.003) and Raynaud’s phenomenon(OR=3.666, 95%CI: 1.076-12.482, P=0.038) were the independent risk factors for positive anti-Ro52 antibody in patients with anti-Jo-1 antibody syndrome. The median survival time was shorter in positive group(24 months) than that in negative group(25 months)(P<0.05). Conclusion The anti-Jo-1 antibody syndrome patients with positive anti-Ro52 antibody have short course of disease. Arthritis and Raynaud’s phenomenon are the independent risk factors for positive anti-Ro52 antibody.
