摘要
目的比较三孔和单孔腹腔镜手术治疗急性阑尾炎伴胆囊结石患者的疗效。方法随机数表法将94例急性阑尾炎伴胆囊结石患者分为三孔组与单孔组,各47例,分别行三孔、单孔腹腔镜手术。比较两组患者的手术情况、术后疼痛评分、手术满意率、并发症。结果单孔组和三孔组手术时间以及术中出血量比较无统计学差异(P>0.05),但是单孔组患者肠胃功能恢复时间以及住院时间明显少于三孔组(P<0.05);单孔组术后疼痛VAS评分明显低于三孔组,患者满意度评分明显高于三孔组(P<0.05);单孔组切口疼痛、脐疝、腹腔感染、皮下气肿等并发症的发生率明显低于三孔组(P<0.05)。结论急性阑尾炎伴胆囊结石患者行单孔腹腔镜术更好,能明显减少住院时间和患者痛苦,减少并发症,促进患者恢复。
Objective To compare the efficacy of three-hole and single-hole laparoscopic surgery for acute appendicitis with gallstones.Methods 94 patients with acute appendicitis and gallstones were randomly divided into three-hole group and single-hole group,47 cases each.Three-hole and single-hole laparoscopic surgery were performed.The operation,postoperative pain score,surgical satisfaction rate,and complications were compared between the two groups.Results There were no significant differences in the operation time and intraoperative blood loss between the single-hole group and the three-hole group(P>0.05).However,the recovery time of gastrointestinal function and hospitalization time in the single-hole group were significantly less than those in the three-hole group(P<0.05).The VAS score of postoperative pain in the single-hole group was significantly lower than that in the three-hole group,and the patient satisfaction score was significantly higher than that in the three-hole group(P<0.05).The incidence of complications such as incision pain,umbilical hernia,abdominal infection,and subcutaneous emphysema in the single-hole group was significantly lower than that in the three-hole group(P<0.05).Conclusion Single-hole laparoscopic surgery is more effective in patients with acute appendicitis and gallstones,which can significantly reduce hospitalization time and patient suffering,reduce complications,and promote patient recovery.
