全麻复合超声引导下胸椎旁阻滞对术后应激反应及VAS评分的影响

摘要 目的探讨全麻复合超声引导下胸椎旁阻滞对择期食管癌开胸手术患者的应用效果。方法选取2017年4月~2018年3月我院收治的72例择期食管癌开胸手术患者,根据随机数字表法分为两组各36例。观察组施行全麻复合超声引导下胸椎旁阻滞,对照组施行单纯全身麻醉,比较两组麻醉前(T0)、插管5min(T1)、术中30min(T2)及术后(T3)血清去甲肾上腺素(NE)及皮质醇(Cor)水平、比较两组术后6h(T4)、12h(T5)、24h(T6)、48h(T7)咳嗽及静息状态下视觉模拟评分(VAS)变化。结果两组T1、T2及T3时血清Cor和NE水平较T0时升高,但观察组较对照组低(P<0.05),观察组T4、T5、T6及T7时VAS评分较对照组显著降低(P<0.05)。结论全麻复合超声引导下胸椎旁阻滞能显著减轻食管癌开胸手术患者应激反应,缓解术后疼痛。
