补阳还五汤联合丁苯酞注射液对急性脑梗死恢复期患者神经功能保护作用的研究

Study on Protective Effect of Buyang Huanwu Decoction Combined with Butylphthalide Injection on Neurological Function in Patients with Acute Cerebral Infarction at Convalescence Stage

目的探讨补阳还五汤联合丁苯酞注射液对急性脑梗死恢复期患者神经功能的保护作用。方法将2016年12月—2018年7月本院急性脑梗死恢复期患者124例作为观察对象,随机分为两组,对照组采用丁苯酞氯化钠注射液200 mL/d治疗;观察组采用补阳还五汤联合丁苯酞注射液200 mL/d治疗,持续治疗2周为1个疗程,共2个疗程。比较两组患者的临床疗效,治疗前后抽血测定血液中肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1(interleukin-1,IL-1)及脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)的水平,记录药物安全性。结果对照组有效率80.65%低于观察组有效率93.55%,差异具有统计学意义(P<0.05)。两组患者治疗前血TNF-α、IL-1及血BDNF比较,差异无统计学意义(P>0.05)。治疗后对照组和观察组血TNF-α(58.41±21.16)pg/mL、(43.08±20.17)pg/mL,血IL-1为(11.09±2.87)ng/L、(8.71±2.64)ng/L,血BDNF(3.14±0.42)ng/mL、(3.60±0.49)ng/mL,对照组和观察组上述指标比较,差异有统计学意义(P<0.05)。两组患者在不良反应上,差异无统计学意义(P>0.05)。结论补阳还五汤联合丁苯酞注射液对急性脑梗死恢复期患者的疗效显著,降低炎症因子,有助于保护脑神经功能,安全性高,值得推广。

Objective To investigate the protective effect of Buyang Huanwu Decoction combined with Butylphthalide Injection on neurological function in patients with acute cerebral infarction in convalescence.Methods A total of 124 patients with acute cerebral infarction in convalescent period in our hospital from December 2016 to July 2018 were randomly divided into two groups.The control group was treated with Sodium Butylphthalide Chloride Injection 200 mL/d.The observation group was treated with Buyang Huanwu Decoction combined with Butylphthalide Injection 200 mL/d.The treatment lasted for 2 weeks as a course of treatment,a total of 2 courses.The clinical efficacy was compared between the two groups.The levels of tumor necrosis factor-alpha(TNF-α),interleukin-1(IL-1)and brain-derived neurotrophic factor(BDNF)in blood were measured before and after treatment.We recorded the drug safety.Results The effective rate of the control group was 80.65%,which was lower than that of the observation group(93.55%)(P<0.05).There was no significant difference in TNF-α,IL-1 or BDNF between the two groups before treatment(P>0.05).After treatment,levels of TNF-αin the control group and observation group were(58.41±21.16)pg/mL and(43.08±20.17)pg/mL,respectively.The serum levels of IL-1 were(11.09±2.87)ng/L and(8.71±2.64)ng/L,respectively.The serum levels of BDNF were(3.14±0.42)ng/mL and(3.60±0.49)ng/mL,respectively.And indexes in the control group and observation group had significant differences(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Buyang Huanwu Decoction combined with Butylphthalide Injection has remarkable curative effect on patients with acute cerebral infarction in convalescent stage.It can reduce inflammatory factors and help to protect brain nerve function.It is safe and worthy of popularizing.
