椎旁肌退变与腰椎间盘突出症关系的研究进展

Research Progress on Relationship between Paravertebral Degeneration and Lumbar Disc Herniation

腰椎间盘突出症是临床常见病,其发病率高、易反复,现对其研究由椎间盘拓展到椎旁小关节、椎体及韧带、椎旁肌肉等。现有研究发现椎旁肌退变与椎间盘突出症的病程、病变节段、临床表现相关,阅片时发现双侧椎旁肌萎缩、脂肪浸润增加,单侧突出者患侧脂肪浸润尤为明显,日久可累及健侧,加重症状。在临床物理治疗中常见患者脊柱双侧对治疗强度刺激敏感度不同,腰肌的不对称性对腰椎间盘突出症的临床诊断、治疗、功能锻炼具有重要意义。故查阅已有研究,明确椎旁肌退变与腰椎间盘突出症的关系,以期指导临床。

Lumbar disc herniation is a common clinical disease with a high incidence rate and easily recurrent.The current study extends from disc to paravertebral facet joints,vertebral and ligaments,paravertebral muscles,etc.Existing studies have found that paravertebral muscle degeneration is related to the course of disease,lesion segment and clinical manifestations of disc herniation.We usually found bilateral paravertebral muscle atrophy and increased fat infiltration in imaging examinations.The unilateral protrusion is particularly obvious in the affected side,which may involve the healthy side over time and aggravate the symptoms.In clinical physical therapy,the sensitivity of the two sides is different,and the asymmetry of the lumbar muscle is of great significance for the clinical diagnosis,treatment and functional exercise of lumbar disc herniation.Therefore,previous studies were reviewed to clarify the relationship between paravertebral degeneration and lumbar disc herniation,so as to guide clinical practice.
