性别和血糖异常情况对精神分裂患者认知功能的影响

摘要 目的考察性别和血糖异常情况对精神分裂患者认知功能的影响。方法选取2018年4月~2019年4月我院收治的100例精神分裂患者作为研究对象,按照是否合并2型糖尿病分为伴糖尿病组(50例,其中男28例、女22例)和单纯组(50例,其中男30例、女20例)。比较两组患者的低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、糖化血红蛋白(HbA1c)及空腹血糖(FPG)的水平,并采用重复性成套神经心理状态测验评估患者的认知功能水平。结果两组患者的LDL-C、HDL-C相比,差异无统计学意义(P>0.05);伴糖尿病组男性和女性的FPG、HbA1c水平高于单纯组男性和女性患者,差异有统计学意义(P<0.05);伴糖尿病组男性的FPG和HbA1c水平高于同组女性患者,差异有统计学意义(P<0.05);伴糖尿病组男性患者的延时记忆、注意、语言、视觉广度及即刻记忆分数低于单纯组男性,差异有统计学意义(P<0.05);伴糖尿病组女性患者的注意及即刻记忆低于单纯组女性,差异有统计学意义(P<0.05);伴糖尿病组男性认知功能分数低于同组女性(P<0.05);单纯组男性的延时记忆、视觉广度及即刻记忆分数低于同组女性,差异有统计学意义(P<0.05);Spearman相关性结果显示:男性伴糖尿病的精神分裂症患者的FPG与注意评分成负相关性,差异有统计学意义(r=-0.32,P=0.000);而与延时记忆、语言、视觉广度及即刻记忆无相关性(P>0.05);女性伴糖尿病的神经分裂症患者的FPG与延时记忆、注意、语言、视觉广度及即刻记忆等认知功能均无相关性(P>0.05)。结论血糖异常患者可能会加重精神分裂症患者的认知功能损伤程度,男性的认知功能损伤更严重。 Objective To investigate the effect of gender and abnormal blood glucose on cognitive function with schizophrenia patients.Methods A total of 100 cases schizophrenia patients were collected from April 2018 to April 2019 in our hospital,according to whether combined type 2 diabetes mellitus,they were divided into with diabetes mellitus group(50 cases,28 cases men,22 cases were women)and simple group(50 cases,30 cases men,20 cases were women).The levels of low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),glycosylated hemoglobin(HbA1c)and fasting blood glucose(FPG)were compared between the two groups.A repetitive set of neuropsychological status tests was used to assess the patients′cognitive function.Results There were no significant differences in LDL-C and HDL-C between the two groups(P>0.05);the levels of FPG and HbA1c in men and women in the group with diabetes were respectively higher than those of the men and women in the simple group,with statistically significant differences(P<0.05);the levels of FPG and HbA1c of men in the group with diabetes were increased than those of the women in the same group,with statistically significant differences(P<0.05);the delay memory attention language visual span and immediate memory score of men in the group with diabetes were lower than those of the men in the simple group,with statistically significant differences(P<0.05);the attention and immediate memory of women in the group with diabetes were lower than those of the women in the simple group,with statistically significant differences(P<0.05);the cognitive function scores of the men in the group with diabetes were lower than those of the women in the same group,with statistically significant differences(P<0.05);the visual span and immediate memory scores of the men in simple group were lower than those of the women in the same group,with statistically significant differences(P<0.05);Spearman correlation showed that fasting blood glucose was negatively correlated with attention in male schizophrenic patients with diabetes,with statistically significant difference(r=-0.32,P=0.000);but there were no correlations between the delayed recall memory,language,visual span,immediate memory and the FPG(P>0.05);There were no correlations between the FPG of the women schizophrenic patients with diabetes and the delayed recall memory,attention,language,visual span,immediate memory(P>0.05).Conclusion Patients with abnormal blood glucose may aggravate the cognitive impairment in schizophrenics,especially in men.
