承气清胰汤对重症急性胰腺炎患者血尿淀粉酶及炎症因子的影响探讨

Influence of Chengqiqingyi Decoction on blood urine amylase and inflammatory factors in patients with severe acute pancreatitis

目的:探讨承气清胰汤治疗重症急性胰腺炎(SAP)的疗效,以及对患者血尿淀粉酶和炎症因子水平的影响。方法:选取医院2016年3月至2019年8月期间收治的SAP患者68例,采用随机数字表法将患者分为对照组与观察组,每组34例。对照组给予单纯西医治疗;观察组联合承气清胰汤治疗,分早晚两次各200 mL,由胃管注入。两组疗程均为2周。比较两组患者病情恢复情况以及血淀粉酶、尿淀粉酶、降钙素原(PCT)、C反应蛋白(CRP)和肿瘤坏死因子(TNF-α)水平,评价疗效。结果:观察组患者腹痛消失时间、肠鸣音恢复时间、首次排便时间均短于对照组(P<0.05);治疗后观察组血淀粉酶、尿淀粉酶、PCT、CRP、TNF-α水平低于对照组(P<0.05);观察组总有效率高于对照组(P<0.05)。结论:采用承气清胰汤治疗SAP,能够显著提高疗效,缩短患者恢复时间,降低炎症因子和血尿淀粉酶水平,值得推广。

Objective:To explore the therapeutic effect of ChengqiQingyi Decoction on severe acute pancreatitis(SAP),as well as its effect on blood urinary amylase and inflammatory factors.Methods:68 patients with SAP who were admitted to the hospital from March 2016 to August 2019 were selected,and the patients were divided into a control group and an observation group with a random number table method,34 cases in each group.The control group was treated with Western medicine alone,and the observation group was treated with Chengqi Qingyi Decoction,which was divided into 200 mL each morning and evening and injected through the gastric tube.The course of treatment in both groups was 2 weeks.The recovery status,blood amylase,urinary amylase,procalcitonin(PCT),C-reactive protein(CRP)and tumor necrosis factor(TNF-α)levels were compared between the two groups to evaluate the efficacy.Results:The time of disappearance of abdominal pain,recovery time of bowel sounds,and time of first bowel movement were shorter in the observation group than those in the control group(P<0.05);after treatment,the levels of blood amylase,urinary amylase,PCT,CRP,TNF-αwere low in the observation group compared with the control group(P<0.05);the total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion:The treatment of SAP with Chengqi Qingyi Decoction can significantly improve the curative effect,shorten the recovery time of patients,and reduce the levels of inflammatory factors and blood and urine amylase,which is worthy of promotion.
