摘要
目的本文旨在运用超声研究江苏地区老年男性人群中尺骨前肌厚度、肌肉质量对上肢肌肉力量的影响。方法选取75例65岁以上的老年人,排除骨骼肌肉疾病、肿瘤、感染、外伤、手术、严重肝肾功能不全的人群检测血脂、白蛋白、血糖、血钙等水平。用握力器测量握力,采用超声检测尺骨前肌肉的厚度以及反映肌肉质量的超声回声强度值(ROI)。分析握力与肌肉厚度及质量的相关性。结果75例老年男性受试者平均上肢握力为(24.11±6.46)kg,进一步对上肢握力与临床参数、肌肉厚度和质量进行相关性分析发现,握力水平与年龄呈负相关(r=-0.45,P<0.01),与白蛋白(r=0.36,P=0.002)、血钙(r=0.25,P=0.04)、LDL-C(r=0.33,P=0.01)均呈正相关;握力与尺骨前肌肉厚度呈正相关(r=0.24,P=0.03),与尺骨前肌ROI呈负相关(r=-0.60,P<0.01)。多元线性回归分析在校正了年龄、身高、白蛋白、血钙、LDL-C这些因素之后,上肢握力与尺骨前肌ROI依然存在相关性(β=-0.585,P<0.01)。结论超声检测可同时评估肌肉厚度和肌肉质量,上肢力量与尺骨前肌的质量密切相关,肌肉质量较肌肉厚度更能反映上肢握力水平。
Objective To investigate the relationship of handgrip strength with the thickness and quality of anterior ulnar muscle detected by ultrasound.Methods A total of 75 elderly men in Jiangsu Province without medications or diseases which obviously affected muscle metabolism were included in this study.Handgrip strength was measured by using a Jamar hand dynamometer.The concentrations of serum levels of triglycerides,cholesterol,albumin,glucose and calcium were determined.The thickness and quality of the anterior ulnar muscle were detected by ultrasound and analyzed by QLAB software.Results The average level of handgrip strength was(24.11±6.46)kg.The handgrip strength was positively related with albumin(r=0.36,P=0.002),blood calcium(r=0.25,P=0.04),low density lipoprotein cholesterol(r=0.33,P=0.01)but negatively related with age(r=-0.45,P<0.01).Correlation analysis also showed that handgrip strength was positively correlated with the thickness of anterior ulnar muscle(r=0.24,P=0.03),but negatively correlated with the quality of anterior ulnar muscle(r=-0.60,P<0.01).Multiple linear regression model analysis showed that handgrip strength was associated with the muscle quality(β=-0.585,P<0.01).Conclusions Ultrasonic test can simultaneously evaluate the muscle thickness and muscle quality.Upper grip strength is closely related to the quality of anterior ulnar muscle in the elderly men in Jiangsu province.Muscle quality can better reflect the muscle strength than muscle thickness.
