康复新液联合蒙脱石散治疗小儿尿布皮炎的疗效评价及抗炎作用分析

Efficacy evaluation of Kangfuxin solution combined with Montmorillonite powder in the treatment of diaper dermatitis and anti-inflammatory effect analysis

目的观察康复新液联合蒙脱石散外用治疗小儿尿布皮炎皮损的效果,并分析其抗炎作用机制。方法回顾性分析2018年4月至2020年3月武汉市第三医院(武汉大学附属同仁医院)收治的112例小儿尿布皮炎患儿的临床资料,按治疗方案不同将其分为两组:对照组和观察组,每组各56例。对照组患儿采用常规对症处理,观察组患儿在此基础上给予康复新液联合蒙脱石散外用。对比两组患儿的临床疗效、症状改善时间及血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-4(IL-4)]的表达。结果观察组患儿治疗的总有效率为96.4%,高于对照组的80.4%,差异具有统计学意义(P<0.05)。观察组患儿治疗后的红斑丘疹、渗液、皮肤糜烂及皮损消退时间分别为(1.14±0.08)d、(2.05±0.37)d、(2.67±0.42)d、(3.71±0.35)d,均分别短于对照组的(2.05±0.75)d、(3.67±0.43)d、(4.59±0.71)d、(5.25±0.82)d,差异具有统计学意义(P<0.05)。观察组患儿治疗后的血清TNF-α、IL-6、IL-4水平分别为(8.69±1.28)ng/L、(62.62±6.71)ng/L、(4.35±0.83)ng/L,均分别低于对照组的(13.31±1.35)ng/L、(79.78±7.93)ng/L、(6.73±0.76)ng/L,差异具有统计学意义(P<0.05)。结论康复新液联合蒙脱石散外用治疗小儿尿布皮炎可有效提高治疗效果,降低炎性因子含量,缩短症状消退时间。

Objective To observe the effect of Kangfuxin solution combined with montmorillonite powder in the treatment of diaper dermatitis skin lesions in children,and analyze its anti-inflammatory mechanism.Methods The clinical data of 112 children with diaper dermatitis admitted to Wuhan Third Hospital(Tongren Hospital of Wuhan University)from April 2018 to March 2020 were selected and the patients were divided into two groups according to different treatment options:the control group and the observation group,with 56 cases in each group.Children in the control group were treated with routine symptomatic treatment,and on this basis,children in the observation group were given Kangfuxin solution combined with montmorillonite powder for external use.The clinical efficacy,symptom improvement time and serum inflammatory factors expression[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-4(IL-4)]between the two groups were compared.Results The total effective rate of treatment for children in the observation group was 96.4%,which was higher than 80.4%in the control group.The difference was statistically significant(P<0.05).The time of erythema papules,exudate,skin erosion and skin lesions subsided after treatment in the observation group were(1.14±0.08)d,(2.05±0.37)d,(2.67±0.42)d,(3.71±0.35)d,respectively.All were shorter than those of the control group's(2.05±0.75)d,(3.67±0.43)d,(4.59±0.71)d,(5.25±0.82)d,and the differences were statistically significant(P<0.05).Serum levels of TNF-α,IL-6 and IL-4 in the observation group after treatment were(8.69±1.28)ng/L,(62.62±6.71)ng/L,(4.35±0.83)ng/L,which were lower than those of the control group(13.31±1.35)ng/L,(79.78±7.93)ng/L,(6.73±0.76)ng/L,the differences were statistically significant(P<0.05).Conclusion Kangfuxin solution combined with montmorillonite powder for topical treatment of diaper dermatitis in children can effectively improve the treatment effect,reduce the content of inflammatory factors,and shorten the time for symptom resolution.
