胰腺神经内分泌肿瘤的影像学表现与临床病理分析

Imaging Manifestations and Clinicopathological Analysis of Pancreatic Neuroendocrine Tumors

目的分析胰腺神经内分泌肿瘤的影像学表现与临床病理对照分析结果。方法选取本院45例胰腺神经内分泌肿瘤患者,均行CT平扫与动态增强扫描,对比临床病理结果,分析CT诊断的符合率与影像学表现。结果 CT检出胰腺神经内分泌肿瘤39例,符合率为86.7%。45例均是单发病灶,直径1~10cm,26例处于胰头,19例例处于胰尾;功能性胰腺神经内分泌肿瘤中,11例CT平扫为等密度结节,动脉期强化明显,高于正常胰实质,8例血管活性肠肽瘤CT平扫呈边缘清楚的略低密度肿块,门静脉期呈邻近区域环形强化,高于胰实质;非功能性胰腺神经内分泌肿瘤中,5例平扫呈囊实性肿块,实性部分动脉期强化明显,门静脉期强化减弱,其余12例CT平扫3例囊性,4例实性、5例囊实性。结论胰腺神经内分泌肿瘤应用CT诊断的准确率较高,且其影像学表现具有一定的特征。

Objective To analyze the imaging manifestations of pancreatic neuroendocrine tumors and the results of clinical pathology.Methods 45 cases of neuroendocrine tumor of pancreas were selected for CT plain scan and dynamic contrast-enhanced scan.Results 39 cases of pancreatic neuroendocrine tumor were detected by CT,the coincidence rate was 86.7%.45 cases were single focus with a diameter of 1-10cm,26 cases were in the head of pancreas,19 cases were in the tail of pancreas;among functional pancreatic neuroendocrine tumors,11 cases were isodense nodules on CT plain scan,with obvious enhancement in arterial phase,which was higher than that in normal pancreatic parenchyma;8 cases were slightly low-density masses with clear margin on CT plain scan,with annular enhancement in adjacent area in portal vein phase,which was higher than that in pancreatic parenchyma;non functional Among the neuroendocrine tumors of the pancreas,5 cases showed solid mass on plain scan,the solid part of which had obvious enhancement in arterial phase and weakened enhancement in portal phase.The other 12 cases had cystic mass on plain CT scan,4 cases solid mass and 5 cases cystic solid mass.Conclusion the accuracy of CT in the diagnosis of pancreatic neuroendocrine tumor is high,and its imaging features have certain characteristics.
