清肝泻火消瘿汤治疗甲状腺功能亢进Graves病临床观察

Clinical Observation on Qinggan Xiehuo Xiaoying Decoction in the Treatment of Hyperthyroidism Graves Disease

目的系统性评估清肝泻火消瘿汤对甲状腺功能亢进Graves病患者的临床治疗效果。方法选取自2018年4月—2019年4月于抚顺市中医院内三科病房住院并接受治疗的甲亢Graves病患者60例,入组病例均采用信封法随机分为对照组和试验组。对照组30例,应用甲硫咪唑口服治疗方案,试验组30例,在对照组基础上联合清肝泻火消瘿汤内服治疗方案,观察两组患者治疗前后中医证候积分、甲状腺体积大小及临床疗效变化情况。结果经临床治疗,试验组甲状腺体积(22.51±2.80)cm3,中医证候积分(6.80±3.21),临床有效率93.33%,对照组甲状腺体积(27.42±3.26)cm3,中医证候积分(11.55±3.51),临床有效率83.33%,试验组均优于对照组,有统计学意义(P<0.05)。结论在常规治疗基础上联合应用清肝泻火消瘿汤内服可有效改善甲状腺功能亢进Graves病患者机体甲状腺激素及促甲状腺素水平,滋阴清肝、养血清热、解郁散结、养血消瘿,恢复患者临床体征,缓解甲状腺腺体肿胀状态,中西药物联合应用临床疗效显著提高,值得临床推广。

Objective To systematically evaluate the clinical effect of Qinggan Xiehuo Xiaoyu decoction on patients with hyperthyroidism with Graves disease.Methods Sixty patients with Graves disease were enrolled.The enrolled patients were randomly divided into control group and experimental group by envelope method.In the control group,30 patients were treated with methimazole orally,and 30 patients in the experimental group were given Qinggan Xiehuo Xiaoyu decoction on the basis of the control group.The changes of TCM syndrome score,thyroid volume and clinical efficacy were observed before and after treatment.Results After clinical treatment,thyroid volume of the experimental group(22.51±2.80)cm3,TCM syndrome score(6.80±3.21),the clinical effective rate was 93.33%;thyroid volume of the control group(27.42±3.26)cm3,TCM syndrome score(11.55±3.51),the clinical effective rate was 83.33%,the experimental group was better than the control group,and there had statistical significance(P<0.05).Conclusion Combined with Qinggan Xiehuo Xiaoyu decoction on the basis of conventional treatment can effectively improve the thyroid hormone and thyrotropin levels in patients with thyroid hyperthyroidism Graves disease,nourishing yin and clearing the liver,raising serum heat,relieving stagnation and nourishing,nourishing blood and eliminating the phlegm,which is worthy of clinical promotion.
