胆碱酯酶与谷氨酸脱氢酶联合检测在肝脏疾病诊断中的临床价值

Clinical Value of Combined Detection of GLDH and CHE in Diagnosis of Liver Diseases

目的探讨在肝脏疾病诊断中联合检测胆碱酯酶(CHE)和谷氨酸脱氢酶(GLDH)的临床价值。方法抽取河南科技大学第一附属医院收治的217例肝脏疾病患者为疾病组,包括慢性肝炎48例,急性肝炎37例,酒精性肝炎52例,肝硬化41例,原发性肝炎39例,另选取120例健康人群作为健康对照组,入组者均进行CHE和GLDH检测,分析各组人群CHE、GLDH检测结果及诊断肝脏疾病阳性率。结果与健康对照组比较,疾病组的CHE水平明显下降,而GLDH水平显著升高(P<0.05)。慢性肝炎组、肝硬化组、原发性肝炎组患者的CHE水平明显低于对照组(P<0.05);急性肝炎组、酒精性肝炎组、肝硬化组、原发性肝炎组患者的GLDH水平显著高于健康对照组(P<0.05)。在慢性肝炎、酒精性肝炎及肝硬化诊断中,CHE和GLDH联合检测的阳性率均高于二者单独检测的阳性率(P<0.05)。结论在肝脏疾病诊断中,联合检测CHE和GLDH具有较高的检出率,有助于各类肝脏疾病的鉴别诊断,可为临床治疗提供科学参考。

Objective To explore the clinical value of combined detection of glutamate dehydrogenase(GLDH)and cholinester ase(CHE)in the diagnosis of liver diseases.Methods The 217 patients with liver disease treated in the hospital were selected as the disease group,including 48 cases of chronic hepatitis,37 cases of acute hepatitis,52 cases of alcoholic hepatitis,41 cases of cirrhosis,39 cases of primary hepatitis,and another 120 healthy patients.The population was used as a healthy control group.All the participants were tested for CHE and GLDH.The results of CHE and GLDH in each group were analyzed and the positive rate of diagnosis of liver disease was analyzed.Results Compared with the healthy control group,the CHE level in the disease group de creased significantly,while the GLDH level increased significantly(P<0.05).CHE levels in patients with chronic hepatitis,cirrhosis,and primary hepatitis were significantly lower than those in the control group(P<0.05);GLDH levels in patients with acute hepatitis,alcoholic hepatitis,cirrhosis,and primary hepatitis.It was significantly higher than the control group(P<0.05).In the di agnosis of chronic hepatitis,alcoholic hepatitis and cirrhosis,the positive rate of combined detection of CHE and GLDH was higher than the positive rate of both alone(P<0.05).Conclusion In the diagnosis of liver diseases,the combined detection of CHE and GLDH has a higher detection rate,which is helpful for the differential diagnosis of various liver diseases and can provide scientific reference for clinical treatment.
