云端锻炼控制系统对稳定期COPD患者锻炼依从性及生活质量的影响 被引量:2

摘要 目的探究云端锻炼控制系统对稳定期COPD患者锻炼依从性与生活质量的影响。方法采用前瞻性研究方法,选择丽水市第二人民医院2017年6月~2019年5月收治的COPD患者160例为研究对象,采用随机数字表法进行随机分组,对照组与研究组各80例。患者病情进入稳定期后,对照组开展常规锻炼;研究组在对照组基础上增加云端锻炼控制系统监督步行,对两组患者进行随访3个月,比较两组患者肺功能(FEV1、PEF、FVC)、生活质量(CAT)、步行情况(6MWD)、锻炼依从性、急性发作次数及呼吸困难程度(mMRC)。结果两组患者出院时FEV1、PEF、FVC评分比较,差异无统计学意义(P>0.05),3个月后研究组各指标水平明显高于对照组,差异有统计学意义(P<0.05);两组患者出院时CAT、6MWD、mMRC数据比较,差异无统计学意义(P>0.05),3个月后研究组CAT、mMRC评分明显低于对照组,6MWD数据明显高于对照组,差异有统计学意义(P<0.05);研究组3个月后锻炼依从性97.50%明显高于对照组的78.75%(P<0.05);研究组患者3个月内COPD急性发作次数明显低于对照组(P<0.05)。结论云端锻炼控制系统能够提升稳定期COPD患者运动锻炼依从性,改善其肺功能,降低呼吸困难程度,提升生活质量,值得应用与推广。 Objective To explore the effect of cloud exercise control system on exercise compliance and quality of life of patients with stable chronic obstructive pulmonary disease(COPD).Methods 160 COPD patients admitted in our hospital from June 2017 to May 2019 were randomly divided into control group and study group,with 80 cases in each group.After the patient's condition entered a stable period,the control group carried out routine exercise,while the study group was given cloud exercise control system to supervise walking on the basis of the control group,and all patients were followed up for 3 months.The lung function[forced expiratory volume in 1 second(FEV1),peak expiratory flow(PEF),forced vital capacity(FVC)],quality of life[COPD assessment test(CAT)],walking condition[6-min walking diistance(6MWD)],exercise compliance,times of acute attacks and degree of dyspnea[modified medical research council(mMRC)]of the two groups were compared.Results There was no significant difference in FEV1,PEF and FVC scores between the two groups when discharged(P>0.05),after 3 months,the level of each index in the study group was significantly higher than the control group(P<0.05).There was no significant difference in CAT,6MWD and mMRC between the two groups when discharged(P>0.05).After 3 months,the scores of CAT and mMRC in the study group were significantly lower than that in the control group(P<0.05),while the 6MWD score was significantly higher than that in the control group(P<0.05).The exercise compliance in the study group(97.50%)after 3-month exercise was significantly higher than that in the control group(78.75%)(P<0.05).The times of acute attacks of COPD in the three months in the study group were significantly lower than those in the control group(P<0.05).Conclusion The cloud exercise control system can improve the exercise compliance of COPD patients in stable period,improve their lung function,reduce the degree of dyspnea,and promote the quality of life,which is worthy of application and promotion.
