非酒精性脂肪肝患者血清脂肪素、内脂素和鸢尾素水平及其相关性

To investigate the levels of adipsin,visfatin and irisin in patients with nonalcoholic fatty liver disease(NAFLD)and their correlation

目的探讨非酒精性脂肪肝(NAFLD)患者体内循环脂肪素(adipsin)、内脂素(visfatin)和鸢尾素(irisin)水平及其相关性。方法选取我院2018年1月至2019年12月门诊及住院的NAFLD患者120例和性别、年龄等一般情况相匹配的正常人120例,采集两组受试者血样,记录并分析其一般生化指标、血清adipsin、visfatin和irisin的浓度水平。测量并计算体质量指数(body mass index,BMI)和胰岛素抵抗指数(insulin resistance of homeostasis model assessment,HOMA-IR)。分析血清adipsin、visfatin和irisin水平与NAFLD发病的相关性。结果NAFLD患者BMI、TG、TC、AST、ALT、FBG、HOMR-IR、adipsin水平较对照组高,HDL-C、visfatin和irisin水平较对照组低(P<0.05)。NAFLD患者adipsin水平与BMI、FBG、FINS、HOMA-IR、TG呈正相关(P<0.05);NAFLD患者visfatin及irisin水平均与BMI、FBG、FINS、HOMA-IR、TG呈负相关(P<0.05)。按四分位法、最高四分位数组与最低四分位数组比较,adipsin、visfatin、adipsin与NAFLD患病风险的OR值分别为4.15(95%CI:1.18~14.66)、0.15(95%CI:0.05~0.53)和0.20(95%CI:0.06~0.71)。结论循环中脂肪素水平的升高、内脂素和鸢尾素水平的降低与NAFLD患病风险相关。

Objective To investigate the levels and the correlation of adipsin,visfatin and irisin in patientswith nonalcoholic fatty liver disease(NAFLD).Methods120 NAFLD patients and 120 normal peopel who werematched in general conditions such as gender and age were selected in our hospital from January 2018 to December2019 in outpatient and inpatient Settings.The levels of adipsin,visfatin,irisin and other biochemical indices in pa-tients with nonalcoholic fatty liver disease(NAFLD)were measured.Body mass index(BMI),insulin resistance ofhomeostasis model assessment(HOMA-IR)were calculated.The correlation between adipsin,visfati,irisin leveland the occurrence of the NAFLD was analyzed.ResultsThe levels of BMI,TG,TC,LDL-C,AST,ALT,HOMR-IR and adipsin in NAFLD patients were higher than those in the control group and the levels of HDL-C,visfatin andirisin in NAFLD patients were lower(P<0.05).The levels of adipsin in NAFLD patients were positively correlatedwith BMI,HOMR-IR and TG.The levels of visfatin in NAFLD patients were negatively correlated with BMI andHOMR-IR,and positively correlated with HDL-C.The levels of irisin in NAFLD patients were negatively correlatedwith BMI and HOMR-IR.According to the quartile method,the OR values of the highest quartile array and the low-est quartile array were 3.76(95%CI:1.27~11.08),0.30(95%CI:0.10~0.91)and 0.30(95%CI:0.10~0.88),respectively.ConclusionThe increasing of circulating adiposin levels and the decreasing of irisin levels were asso-ciated with the increasing risk of NAFLD.
